The Healthcare Program at Serenity Recovery Connection serves as a cornerstone of our agency, enabling healthcare staff and clinicians to gain a profound understanding of the lived experiences associated with substance use disorders that extends beyond their clinical education. Through this program, they have not only broadened their knowledge but have also discovered a multitude of pathways and resources for patient recovery.


Our non-clinical Healthcare Program is supported by Certified Peer Recovery Coaches who have undergone comprehensive training in the advanced Jordan Peer "Coaching in a Healthcare Setting" curriculum. These dedicated coaches devote the majority of their time within local safety net clinics and healthcare settings. They will also extend their support to adult patients referred from hospitals to include CommonSpirit (formerly Centura.)


The primary purpose of our healthcare coaches is to provide recovery support to patients suffering from various conditions related to substance use disorders, including overdose, alcohol poisoning, and other associated issues. By sharing their own experiences of recovering from a substance use disorder and planting seeds of hope, they instill optimism and inspire individuals on their own journey to recovery.  The ideal candidate will also be familiar with the communities they will be providing services in.  These areas include Fountain, Colorado and/or Calhan, Colorado, therefore primary consideration will be given to people who are currently residing in or have a strong familiarity with these areas of Colorado.


Even after discharge from clinic or hospital, our program continues to extend support to patients through telephone recovery support, peer coaching, and a wide range of resources. We remain committed to empowering individuals beyond their healthcare-setting stay, helping them navigate the challenges and embrace a life of recovery.

General Peer Recovery Coach Job Description

$20-21.50/hour DOE & Certifications

P/T & F/T– Non-Exempt

 

 ESSENTIAL JOB FUNCTIONS

  • Provide direct ongoing coaching to individuals as assigned on a regular basis (face-to-face, on the phone, and virtually)
  • Provide Telephone Recovery Support for assigned Recoverees and assists with TRS Program call list as needed.
  • Will support front office staff as needed, to include answering the phone or sitting at front desk.
  • Ensures assigned paperwork is completed and maintained in accordance with SRC policies and funding sources.
  • Demonstrates and respects that there are multiple pathways to recovery.
  • Consistently enters data and outcomes tracking in SRC database & utilize:
    • Regular Brief-Check-ins as described by SRC SOP,
    • Recovery Capital Assessments as recommended & on schedule,
    • Engagement Logs, and completes a
    • Recovery Management Plan for all participants and engages Recoveree’s in regular follow-up.
  • Required to log all data and document all information, as per SRC SOP in SRC database accurately and thoroughly. Also effectively explains data collection procedures to participants for established metrics and outcomes.  This may include uploading files into database.
  • Refers Family members to SRC Family Advocate.
  • Be aware of all SRC’s mutual aid groups and training offerings. Be able to articulate core components and benefits of program offerings.
  • Ability to refer participants to appropriate financial, housing, transportation, hygiene, employment, nutrition etc.
  • Facilitate timely referrals and placements from hospital to treatment, sober living, or recovery support providers.
  • Work cooperatively and effectively with personnel of assigned site(s) i.e., emergency room or alternative assigned location.
  • Participate in ongoing recovery coach training/education, professional development, performance support and attend weekly and monthly assigned meetings as required.
  • Ability to adjust hours of work based on SRC’s and community needs.
  • Ability to work independently as well as in a team environment.
  • Must meet the hospital requirements to provide services on site (Healthcare Specialty Coaches Only).
  • Consistently and accurately completes timesheets.
  • Provides shadowing and training for new volunteer, intern and staff coaches.
  • All other duties as assigned.

 

EDUCATION

  • High school diploma or graduate equivalent degree required.
  • Certificate of completion of the Complete Peer Coach Academy within 6 months after hire.
  • Demonstrate a willingness to receive additional education and/or certifications to include those provided by SRC.


OTHER QUALIFICATIONS

  • Must be a person with lived experience with substance use disorder. Must have a sustained recovery of two years or more (Subject to the discretion of the CEO).
  • Demonstrate an awareness of critical SUD topics:
    • Harm Reduction, Naloxone Training, Community Resources, and Disease Prevention.
  • Demonstrate knowledge of key success areas:
    • Self-Care: (recovery support, spiritual, emotional, physical, social, mental, professional)
    • Interpersonal Skills: (distress tolerance, leadership style, self-confidence, receptive to feedback, positive attitude).
    • Time Management: (use of calendar, scheduling, looking ahead, communicate with supervisor and schedule time off appropriately).
    • Stewardship: (using resources wisely, thinking through decisions that affect the organization and resources available).
  • Must possess knowledge and understanding of Recovery Oriented Systems of Care, Trauma Informed Services and Motivational Interviewing.
  • Perform job duties and responsibilities in a professional manner at all times and comply with the Code of Ethics. standards and agency norms established for the profession as set forth by Springs Recovery Connection.
  • Participate in quarterly goal setting with supervisor.

Healthcare Program Specialty Coach

Reports to Healthcare Program Manager

 


QUALIFICATION OF ASSIGNED ROLE:

  • Lived experience with Substance Use Disorder.
  • Preferred lived experience as a patient in a healthcare setting, due to substance use disorder.
  • Must have 2+ years continuous sustained recovery.


EDUCATION REQUIREMENTS OF ASSIGNED ROLE:

  • Certificate of Completion from the Jordan Peer Healthcare Settings training within 2 months of hire date.
  • HIPAA Certification within 1 week after hire date
  • Complete Mandatory Report Training within 1 week after hire date.


ESSENTIAL JOB FUNCTIONS OF ASSIGNED ROLE:

  • COVID/Flu vaccination and proof is possibly required to work in this role- dependent on requirements of placement.
  • PRC assigned to this role will work directly under the Healthcare Program Manager.
  • Work collaboratively with SRC staff and Healthcare Program Manager to achieve Healthcare program goals.
  • Work collaboratively with healthcare partners to achieve program goals.
  • Comply with all SRC & healthcare partners Data Reporting requirements.
  • Administers intakes for new Healthcare Program Recoverees.
  • Responsible for making TRS calls to Healthcare Program Recoverees.
  • Attends weekly ZOOM or in-person Healthcare program meetings and follows-up.
  • Recruit volunteers through the Healthcare Program to become coaches at SRC.
  • Facilitate Peer Lead Mutual Aid Support Groups as necessary.
  • All other job duties as described in the Peer Recovery Coach job description.


PHYSICAL REQUIREMENTS 

  • High comfortability with computer-based programs and able to enter and retrieve data utilizing SRC’s computer system application, printers or other office equipment, ZOOM and telephone systems.
  • Must have personal reliable transportation to travel to rural areas for work related activities. Registration, Insurance and valid Driver License required.
  • Must be able to walk, will be required to go up/down stairs and will need to be able to move chairs or other items under 20lbs.
