GENERAL DESCRIPTION
The Social Assistance Navigator is a member of the Learning Community Center of North Omaha’s (LCCNO) navigator team. The Social Assistance Navigator works with participant families to connect them with a variety of social services and community resources. The position requires a working knowledge of the application of principles, methods, and techniques of social work/human services to the solution or alleviation of problems of individuals and/or families. The position requires cross-training as an Educational Navigator and will also perform those duties as needed.
JOB-SPECIFIC DUTIES & RESPONSIBILITIES
1. Participant Case Management: Assesses the needs of participants and their families and connects them with community resources. Applies the principles and practices of social work in the performance of assignments such as assessing, developing and providing direct services or programs to individuals, families, and/or groups. Is the point of contact for participants who present with any type of abuse or neglect. Conducts screenings and education for participants related to their health and well being. Updates family’s progress through the database, adding relevant notes, files, evaluations, attendance, etc. in a timely manner. Helps parents overcome any barriers to fully engaging in the program.
2. Team Dynamics: Uses effective interpersonal skills in working with and receiving referrals from the various teams of the Learning Community Center of South Omaha. Serves as a resource to the navigator team and to the larger team in managing complex psychosocial issues and accessing community resources.
3. Connection to Community Resources: Maintains an ongoing current file of community resources and programs. Assures that LCCSO participants and families are knowledgeable of available community resources and how to access them. Assists participants with formal and informal referrals, phone calls, and completion of paperwork and applications. If time allows, coordinates field trips for small groups to visit organizations in the community that provide needed services for participant families.
4. Outreach Services: Recruits families throughout the community who are interested in the program and screens them to determine program eligibility. Immerses self within the community and school fabric in the catchment area of Subcouncil 2 to build rapport and relationships. Represents the Learning Community Center program to individuals and potential partners in the community. Functions as a liaison between the program and the community at large. Coordinates and presents program information at community events, neighborhood meetings, schools, etc. to create awareness of the Learning Community Center and its services.
5. Educational Navigator Duties: Cross trains as an Educational Navigator to fill in when needed. Completes intake forms and conducts in-home evaluations with each parent participant on an ongoing basis. Regularly visits participant families in their homes and assists them through an evidence-based home visiting practice. Assists parents in setting goals for themselves and their children. Develops and coordinates individual and group educational plans for the parents and their young children. Facilitates parent workshops as requested and identifies potential future parent workshop topics.

QUALIFICATIONS
1. Bachelor’s degree OR equivalent training related to social work, human services, social
services, psychology, sociology, or a related field required. Bachelor’s degree in Social Work
preferred.
2. One to three years of experience in social work, within a healthcare or social
services environment, preferred.
3. One to three years of experience in completing needs assessments and evaluations
preferred.
4. Proficiency and experience with Microsoft Office products required, with a willingness to
learn other programs.
5. Proficiency in both English and Spanish (written and verbal) is required.
6. Strong communication skills (written and verbal) required.
7. Demonstrated task and detail orientation, along with excellent customer service and
organizational skills.
8. Demonstrated ability to self-motivate and self-direct, but also to work as a member of a
team.
9. Demonstrated knowledge of the Latino culture with a commitment to cultural competence.
10. Demonstrated familiarity with social service resources in the community.
11. Ability and willingness to complete home visits. Valid driver’s license and automobile
insurance required.
