OVERVIEW OF THE ROLE/JOB

The primary responsibility of your role in your assigned location is to provide nursing support and clerical duties with supervision of day-to-day nursing activities/responsibilities following the current federal, state, and local standards, guidelines, and regulations that govern long-term care and skilled nursing facilities. You are entrusted to ensure that the highest degree of quality care will always be provided to our residents.


Care Coordination Duties: 100% time
  • Perform clerical duties using specific knowledge of medical terminology, International Classification of Diseases-Tenth Revision (ICD-10) coding, laboratory procedures, and nursing policies and procedures.
  • Transcribe provider orders as permissible by state regulations and assist medical staff during rounds and visits.
  • Facilitate the distribution or routing of hardcopy and/or digital documents; prioritize tasks based on acuity or urgency of the need (i.e., contacting physician's office, etc.)
  • Assist the Charge Nurse/Nurse Supervisor with planning day shift events including physician's rounds.
  • Answer and route incoming telephone calls, take messages, check voicemail, respond to emails, and facilitate communication internally and externally.
  • Back-up in arranging resident appointments for diagnostic and therapeutic services.
  • Operate office equipment such as voice mail messaging systems and use word processing, spreadsheet, or other software applications that promote communication.
  • Greet Visitors, assist with directions (i.e., resident room numbers, etc.), and/or routing visitors, contractors, and vendors to appropriate staff.
  • Conduct documentation/chart reviews periodically. Report discrepancies found to Charge Nurse/Nurse Supervisor.
  • Forward new diet orders and/or diet changes (including food preferences, dislikes, allergies, etc.) to the Certified Dietary Manager, the Registered Dietitian and/or the interdisciplinary team (IDT) as appropriate.
  • Record vital signs, weights, and heights in accordance with facility protocol and state standards of practice.
  • Ensure timely response to call lights by nursing team members; assist with answering call lights as needed.
  • Report administrative problem areas to the supervisor and assist in developing and implementing corrective actions.
  • Attend various committee meetings of the facility (i.e., infection prevention and control, policy advisory, pharmaceutical, QAPI, etc.) as required.
  • Participate in all fire safety and disaster preparedness drills safely and professionally.
  • Obtain PT/INR results and communicate to provider to obtain new orders
  • Cleansing of respiratory equipment in accordance with facility
  • Follow established procedures in the use and disposal of personal protective equipment (PPE)
  • Follow established infection prevention and control procedures; notify the infection preventionist
  • Follow established safety regulations and manufacturers recommendations including the cleaning and maintenance of equipment and supplies.
  • Inventory and stock supplies as directed including patient education tools and resource guides for residents, visitors, or staff.
  • Assist with resident admission, transfer, and discharge functions as directed.
  • Perform administrative duties by completing medical forms, reports, evaluations, studies, charting, etc.
  • Provide nursing services to residents in accordance with scope of practice, facility protocols, and standards of care.
  • Provide residents with care that is compassionate and sensitive to residents with cognitive decline, memory loss, or a history of trauma.
  • Assist with resident meals, including delivering meals and assisting residents with intake as needed.
  • Promote the facilities fall prevention practices to maximize resident safety.
  • Administer medications within the scope of practice and according to practitioners' orders. Report adverse consequences, side effects or any medication errors.
  • Help bed-bound or chair-bound patients turn/reposition as scheduled.
  • Monitor the skin health of residents, provide preventative skin care, administer wound treatments as ordered.
  • Collect and submit lab specimens as ordered.
  • Cooperate with other personnel to achieve department objectives and maintain good employee, interdepartmental, and public relations.
  • Contribute to the orientation, training, and development of new team members as requested.
  • Attending continuing education and in-service training programs as required to provide person-centered and competent care.

  • May be subject to the handling of and exposure to hazardous chemicals.
  • Maintains a liaison with residents, their families, support departments, etc, to adequately communicate and plan for resident's needs.

SKILLS AND KNOWLEDGE:

  • Knowledge of computer systems including Point Click Care
  • Working knowledge of long-term care and skilled nursing state and federal regulations (preferred)
  • Knowledge of nursing and medical practices, procedures, and standards of care
  • Leadership and supervisory skills-willingness to work harmoniously with and supervise other personnel
  • Patience, tact and a cheerful disposition, enthusiasm as well as willingness to handle difficult people
  • Knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served

EDUCATION AND EXPERIENCE:

Experience: One-year experience in a long-term care environment (preferred)

License: Current, unencumbered, active license to practice as an LPN in Colorado, CPR Certification


PHYSICAL REQUIREMENTS:

  • Ability to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility
  • Meet the general health requirements set forth by the policies of the facility which include a mental and physical examination
  • Possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
  • Ability to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum of 50 feet
  • Ability to assist in the evacuation of residents during emergency situations

WORK ENVIRONMENT:
