**Job Description:**
We are currently seeking a dedicated and experienced Registered Nurse (RN) or Licensed Vocational Nurse (LVN)to join our home health and hospice team. As an RN, you will be responsible for providing skilled nursing care, assessing patient needs, and coordinating care plans to ensure the highest quality of care for our patients.

**Responsibilities:**
- Conduct comprehensive patient assessments to develop individualized care plans.
- Administer medications, perform wound care, and provide skilled nursing interventions.
- Collaborate with the interdisciplinary team to coordinate and manage patient care.
- Educate patients and their families on health management and self-care techniques.
- Monitor and evaluate patient progress, adjusting care plans as necessary.

**Qualifications:**
