**Job Summary:**
The Registered Nurse (RN) in the office is responsible for providing administrative support and coordinating patient care services under the direction of the clinical management team. This position requires strong organizational skills, attention to detail, and the ability to communicate effectively with patients, caregivers, and healthcare professionals.
**Responsibilities:**
1. **Patient Intake and Assessment:**
- Receive and review patient referrals from healthcare providers, hospitals, and other sources.
- Conduct initial patient assessments over the phone to gather necessary information for admission.
- Coordinate with field nurses to schedule initial assessments and establish care plans.
2. **Care Coordination:**
- Collaborate with clinical staff to develop individualized care plans for patients based on assessment findings and physician orders.
- Coordinate patient schedules, including visits from field nurses, therapists, and other healthcare professionals.
- Communicate regularly with patients, caregivers, and healthcare team members to ensure continuity of care.
3. **Documentation and Record Keeping:**
- Maintain accurate and up-to-date patient records in accordance with regulatory requirements and agency policies.
- Document all patient interactions, assessments, and care plans in electronic health record (EHR) systems.
- Prepare and submit reports as required by regulatory agencies and insurance providers.
4. **Insurance Verification and Authorization:**
- Verify patient insurance coverage and obtain necessary authorizations for services.
- Communicate with insurance companies to resolve billing and coverage issues as needed.
5. **Quality Assurance and Compliance:**
- Ensure compliance with all federal, state, and local regulations governing home health services.
- Participate in quality assurance activities, including chart audits and performance improvement initiatives.
- Identify areas for process improvement and recommend changes to enhance efficiency and patient satisfaction.
6. **Education and Support:**
- Provide education and support to patients and caregivers regarding their care plans, medications, and self-management techniques.
- Serve as a resource for patients and families, answering questions and addressing concerns as they arise.
**Qualifications:**
- Current licensure as a Registered Nurse (RN) in the state of practice.
- Minimum of 2 years of clinical experience, preferably in home health or a related field.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Proficiency with electronic health record (EHR) systems and other software applications.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of Medicare, Medicaid, and private insurance guidelines for home health services.
