Position Summary:
Resource Facilitation is a partnership that helps people and communities choose, get, and keep information, services, and supports to make informed choices and meet their goals. Resource Facilitation can also be referred to as case management, case coordination, service coordination, or resource coordination.

The Resource Facilitator (RF) supports persons living with brain injury, their families, state agencies, medical and allied health team personnel, employers and other providers in achieving optimal, timely and durable outcomes specifically regarding, but not limited to effective long-term employment retention as well as ensures customer satisfaction and outcome attainment without discrimination by race, creed, culture or financial resources.

The RF also serves as an advocate for the person living with a brain injury, their family and their support team to ensure the program participant’s needs and desires are communicated effectively and appropriately to those involved in care and service provision for the participant.

The Lead RF will provide mentoring and training to an assigned team of RFs, with an emphasis on the provision of field instruction and caseload oversight. Will review, critique and manage fidelity in progress notes, goal worksheets and monthly summaries of assigned RFs. The lead RF will provide direct resource facilitation services to an assigned caseload of consumers.

Key Elements of Resource Facilitation:

  • Focus on early vocational intervention.
  • Identify concerns, needed services and residual impairments that may interfere with vocational reintegration and refer for appropriate medical rehabilitation and community-based services.
  • Integrate vocational goals with rehabilitation therapy goals.
  • Develop comprehensive return-to-work plans that address issues ranging from number of hours worked to the work environment to accommodation and compensatory techniques.
  • Improve community agency linkages to develop a team approach.
  • Provide a smooth transition from medical to community-based services.
  • Use vocational and on-the-job evaluations to gather the best information about a person’s work skills and abilities.
  • Provide appropriate support during work evaluations and after placement often working closely with employment specialists and employers.
  • Provide reasonable work accommodation before the client starts a job.
  • Provide BI education to employers, coworkers, and community service providers, acting as a brain injury resource person for the client, employer and other providers.
  • Provide regular, frequent follow-up after placement until discharged from Resource Facilitation Services.

Essential Functions for Lead Role:

  • Assists with field-based instruction for assigned RFs.
  • Assists in the development of training plans for new staff and provides oversight of training for existing staff.
  • Facilitates staffing of RF clients and meets with assigned RFs outside of regular staffings to discuss difficult cases, brainstorm ideas and solutions, exchange information about resources and contacts and provide training.
  • Maintain staffing spreadsheet numbers and details.
  • Keeps director apprised of progress and performance of team members.
  • Provides input at 90 day and annual performance reviews for assigned team members.
  • Provides assistance with special projects and assignments as requested.
  • Assists team with VR billing system needs and training.
  • Assists with caseload assignments.

Essential Functions for Resource Facilitators:

  • Facilitates and monitors the referral process to state agencies including the Bureau of Vocational Rehabilitation Brain Injury (BI) Specialists, as needed, to promote timely service.
  • Contacts all community generated and vocational rehabilitation referrals of person living with brain injury or their caregiver/representative where appropriate, within two working days and perform introductory functions.
  • Reviews medical information and other pertinent records provided, and requests needed documents and records.
  • Acts on behalf of the program participant and organization as to privacy of medical and social information.
  • Meets with participant, and caregiver/family member whenever possible, within one week of initial contact to review medical history, discuss program information and outcome goals. RF also ensures completeness and accuracy of information and acquires all necessary data required for service provision and program evaluation.
  • Assesses to identify current status, needs, and resources in life domains and evaluates through subjective reports from participants, effectiveness of what has been utilized in the past.
  • Administers and scores psychological/neuropsychological assessments and provides detailed behavioral observations.
  • Plans and documents mutually agreed upon, person-centered goals and service/resource needs and identifies community-based resources for services and supports.
  • Facilitates access to resources through education and advocacy, while proactively helping the individual attain needed services and supports (e.g. brain injury, instrumental, community) based on the plan established by the Resource Facilitation team and seeking supervision when necessary to assist in determining what service needs from the public or private sector the consumer and family may require.
  • Updates clinical management tools, service log systems and databases with accurate and timely information.
  • Proactively monitors the status of the plan as well as the quality and appropriateness of the services and supports, and resources through telephone, Internet contact or personal contact when possible. Contact with either the caregiver or the individual with a brain injury, at a minimum of, every 2 weeks.
  • Provides brain injury education and training to individuals with brain injury and their families/caregivers as well as other state agencies, employment specialists and other providers.
  • Education will include information on brain injury, personal advocacy and partnership development and available resources and how to access them.
  • Promotes TBI-informed care with other providers, including education and training in the utilization of brain injury accommodations specific to each participant and/or generally.
  • Ensures collaboration, integration and coordination between providers and community-based resources.
  • At times it may be necessary to travel to a participant’s home community to explore and secure resources, to educate, to train and to advocate.
  • Attends to and resolves customer complaints by utilizing facility resources to ensure satisfactory resolution and communicates status of complaint resolution to the Program Director who will then communicate the concerns to the Executive Leadership Team.
  • Participates in continuous quality improvement and observes measures to ensure CARF and other quality standards and regulations are met.
  • Uses appropriate technique in performing duties to avoid or minimize accidents and injury related to unsafe practice.
  • The Resource Facilitator is focused on providing customer satisfaction (including, but not limited to consumers, co-workers, visitors, vendors and physicians) as a primary goal/outcome of all interactions; places the consumer first in the performance of job duties and responds to needs in a timely manner as evidenced by the absence of legitimate customer complaints.
  • Other duties as assigned.

Education, Licenses, or Certifications:

  • Bachelor’s Degree from an accredited program in health care or human service field required; Master’s degree preferred.
  • Successful candidates are expected to become certified as a Brain Injury Specialist after one year of experience.
  • Valid Indiana driver’s license, proof of insurance for vehicle, and ability to drive.

Knowledge, skill, ability:

  • Possess an understanding of brain injury treatment, recovery and barriers faced by persons living with brain injury and their families.
  • Must be professional and demonstrate ability to communicate effectively with diverse populations in both written and verbal modalities.
  • Must be empathetic and possess strong relationship building skills
  • Must be highly responsible, reliable and able to prioritize tasks.
  • Must be organized and adept at time management and independent problem solving with strong attention to detail.
  • Skilled in person-centered techniques and the spirit and principles of Motivational Interviewing.
  • Competent in needs assessment and service planning.
  • Knowledge of rehabilitation principles, case management, and those of interdisciplinary team coordination are required as well as a working knowledge of community resources.
  • Ability to be flexible and work with clients who have a variety of physical, cognitive and emotional impairments and concerns.
  • Extremely comfortable using computers, internet, cell phone, printer, scanner, copier and fax machines, and automobiles.

Experience:

  • One to three years experience working as a Resource Facilitator.
  • Prior experience in a lead or management role is preferred.
  • Experience in navigating post-acute private and public sector systems of support as well as managing barriers from a consumer or advocate perspective is preferred.
  • Experience or equivalent experience in healthcare in a collegial role or experience in the field of physical rehabilitation, or case management preferred.

Work schedule:

This is a remote position that generally works Monday thru Friday, 8 a.m. to 4 p.m. with flexibility to meet program participant needs. Some weekend or evening hours may be occasionally required. Quarterly in-person team meetings are required and are held in Easterseals Crossroads headquarters in Indianapolis, IN.
