Summary of the position: The Mobile Med Program Manager (RN) will oversee the overall operation of the Mobile Medication staff.

Description of Duty

Supervision/Consumer Services

  • Direct management and supervision of the daily functions of the department.
  • Provide and document weekly face-to-face group supervision. This will include a combination of clinical supervision, overseeing program design, adherence to Policy and Procedures, referral agency visits, and traveling with the MMP staff.
  • Meet with direct supervisor at least 1x per month to review program issues, revenue, program numbers, communicate changes, discuss outcomes, and explore program/personnel needs.
  • Oversee staff working hours and consumer coverage without previously authorized overtime.
  • Review all daily progress notes, required documentation, and medical record on a regular basis. Ensure that medical records are accurate, current, and meet program standards.
  • Problem-solve and process clinical issues, performance issues and program issues with staff in regular supervision and on as-needed basis.
  • Collaborate closely with staff to ensure that all insurance is confirmed prior to beginning service, that initial and continued stay authorizations are obtained, and that clinical documentation shows consumer meets medical necessity criteria.
  • Complete initial nursing assessments and on-going assessment required in the implementation of consumer care. Develop comprehensive treatment plans for those served to address physical and behavioral health needs using recovery tools and principles.
  • Address physical health care needs of consumers and taking necessary steps to resolve identified needs, including appropriate referrals to primary care.
  • Ensure appropriate medication administration and compliance with agency nursing policy regarding medication management. Administer medications as prescribed for consumers following the scope of nursing practice standards.
  • Develop, supervise, and review consumer progress in self-administration of medication. Consult with the consumer treatment team to develop transition plans to traditional services as consumers complete treatment goals.
  • Provide training to consumers and staff regarding health and hygiene methods, infection control procedures, and signs and symptoms of illness.
  • Review clinical documentation prior to billing.
  • Evaluate nursing activities to ensure patient care and safety.
  • Serve as the program contact person with the pharmacy services used by consumers in the program. Resolve issues with pharmacy services.
  • Ensure that incident reports are thoroughly completed and filed in a timely manner.
  • Ensure that all Regulatory and Accreditation Standards are met in the department.

Scheduling

  • Provide coverage for staff absences and attend stakeholder meetings as needed.
  • Ensure staff availability for weekly supervision and training.

Referral Coordination

  • Meet with referral agencies at least 1x/month. Be aware of changing agency personnel and ensure all staff are informed.
  • Maintain regular contact with referral agencies to provide vacancy updates as well as monitor any waiting lists.

Supplies/Equipment

  • Order supplies and equipment for the program while being aware of the MMP operating budget and functioning within its limits.

Training

  • Provide extensive and on-going support and training for staff around clinical practice, policy and procedures, cell phone use, monthly caseload changes/billing, and other required tasks.
  • Ensure the training compliance of staff.
  • Complete scheduled required annual training.
  • Train newly hired staff.

Marketing

  • Market the MMP to ensure expansion.
  • Develop and maintain collaborative community relationships. Interact with stakeholders in a professional manner.

Human Resources

  • Responsible for hiring and termination of program staff in conjunction with Regional Director – West, Director, and Human Resources.
  • Complete performance evaluations.
  • Ensure that new employee Probational Period and annual evaluations are thorough and given in a timely manner as well as collaboratively developing individual professional development goals.
  • Consult with Human Resources for employee issues and dismissal actions.
  • Review all timesheets to ensure they are properly completed and submitted in a timely manner.
  • Work as a member of a team including being professional, supportive and friendly to staff, colleagues, and supervisors. Keep personal problems away from work. Assist in duties as part of a disciplinary team, assisting staff and colleagues when requested.
  • Provide an appropriate role model for staff and the individuals they serve in appearance, actions, manner of speech, etc.
  • Treat subordinates with courtesy and respect using good principals of management.

Program Development

  • Develop and monitor quality initiatives in the department.
  • Actively participate on GJR committees when requested.
  • Facilitate medical assistance applications and maintain correspondence with County Assistance Office.
  • Monitor and correct eligibility issues for all consumers via the Eligibility Verification System daily.
  • Maintain consumer stipend fund records.
  • Conduct internal audits of consumer charts monthly.
  • Monitor and maintain all outcomes data, including pre and post test.
  • Adhere to, develop and enforce personnel policies and procedures.

Working Conditions: Must be able to lift a minimum of 10 pounds, must be able to walk, stoop, and bend, must be able to work in the community and may be required to meet with clients in their homes in the community.

REQUIREMENTS

Education

Bachelor’s of Science in Nursing

Licensure

Current Pennsylvania Registered Nursing License

Years of Experience

Minimum of:

  • 3 years nursing experience
  • Minimum 1 year psychiatric nursing experience serving seriously mentally ill adults

Computer Experience

Basic skills in Microsoft Word and Excel.

Other

  • Act 33/34 clearances as provided by Child Protective Services Law 23 PA C.S. 16301 and FBI Clearances, Adam Walsh Act 147.
  • Valid PA State driver’s licensure
