Become part of the Social and Community Support Services team as a City of Everett Community Supports Case Manager at Bridgeways -- a social enterprise that provides services and supports to promote quality-of-life, facilitate growth, independence, and a sense of community for individuals with behavioral health conditions.


This is an extremely rewarding position where you can directly help people struggling with behavioral health concerns by working closely with a variety of teams and partners in order to create and follow through with care plans created for clients.


Type & Pay: Full Time, non-exempt, paid biweekly. Salary range from $22.75 - $26.50 per hour, DOE

Requirements:

SUMMARY of the CoE Community Supports Case Manager:

Provides direct services with individuals who are unhoused or at risk of homelessness. Provides referrals to behavioral and physical health and other community and social services providers to assist clients in securing and sustaining housing while also ensuring clients have a continuity of care by providing case management and follow-up services.


ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not limited to:

  • Utilizing a strengths-based approach, assesses the skills and needs of recipients in order to secure and maintain housing. Areas to assess includes and are not limited to behavioral and physical health, finances, social/natural support system, life skills, and the ability to maintain a household.
  • Establishes a client-driven plan with outcome of sustained housing. Plans will outline goals, interventions, and services needed that address needs identified in the initial assessment.
  • Consistently assess individuals' progress and makes necessary adjustments to the client’s care plan to ensure they meet evolving needs.
  • Maintaining accurate and up-to-date records of assessments, care plans, and interventions.
  • Engaging in the behavioral health and homelessness service delivery systems, as well as governmental entitlement programs.
  • Understanding and practicing safekeeping of client-protected health information per Bridgeways’ HIPAA policies and procedures.
  • Representing Bridgeways in community and regional activities, committees, and work groups as requested.
  • Establishing liaisons and relationships with appropriate community agencies to meet necessary current and future housing needs and other supports.
  • Staying abreast of WA State landlord-tenant law, screening laws and procedures, and other appropriate laws and ordinances pertaining to housing regulation.

EDUCATION and/or EXPERIENCE

  • Bachelor’s degree related to the behavioral health or social services field OR;
  • Peer Certification through the Washington Health Care Authority.
  • Two years’ experience working in a social service agency providing case management services, particularly with adults with chronic and persistent mental and behavioral health concerns OR;
  • Lived experience congruent with the Target Population for this service may substitute for the social service agency requirement. Lived experience includes and is not limited to being a veteran, having a behavioral health issue, and/or history of homelessness.

Other Working Knowledge Needed:

  • Familiarity with local, state, and federal low-income housing resources, community resources, and local homeless programs.
  • Knowledge of the behavioral health and homelessness service delivery systems.
  • Knowledge of local, state, and federal housing guidelines, trends, and funding opportunities.
  • Knowledge of Landlord/Tenant Law, Fair Housing Laws, Homeless Management Information Systems, and community housing resources.

OTHER SKILLS

  • Language: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents. Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors. Able to understand and contribute to grant requests for housing funding.
  • Mathematical: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Reasoning: Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to understand broad conceptual terms and specify fine details of concepts used in developing and implementing program, policies, and procedures.



CERTIFICATES, LICENSES, REGISTRATIONS current or able to be obtained within 90 days of employment:

  • Valid Washington State Driver’s License.

OTHER SKILLS AND ABILITIES

  • Ability to promote housing services offered by Bridgeways in community settings.
  • Ability to learn and utilize electronic medical records for confidential and safe record-keeping that conforms to agency requirements.
  • Must agree to and be able to pass a background, OIG, and driver’s record check.

WORK ENVIRONMENT

The employee works in an office space provided in an administrative office environment. The noise level in either work environments is usually moderate. The employee also performs outreach in the community, to other social service and governmental agencies, and to locations where individuals experiencing homeless congregate and live including locations that are not suitable for human habitation.
