Key Responsibilities:
  • Conducts initial evaluations and ongoing assessments of patient conditions.
  • Records patient case details in a comprehensive database.
  • Performs monthly chart reviews and audits as necessary.
  • Contributes to monthly case discussions by sharing relevant patient information.
  • Collaborates with the Program Director to develop and review personalized care plans for patients.
  • Addresses patients' medical needs by consulting with their healthcare providers and coordinating treatment plans.
  • Enhances understanding of medical issues affecting the target demographic.
  • Assists patients in accessing necessary entitlements, resources, and psychosocial referrals.
  • Engages in Quality Assurance and Utilization Review activities as directed.
  • Empowers patients to participate in their care planning decisions.
  • Maintains accurate and timely patient records that are easily accessible for review and comply with all requirements.
  • Fosters collaborative relationships within and between facilities to achieve care objectives.
  • Advocates for patients regarding the accessibility of services.
  • Adheres to State and County regulations governing nurse case management programs.
  • Participates in outreach initiatives aimed at the target population as directed.
  • Recommends enhancements to programs or services to address community service gaps.
  • Performs additional duties as assigned.

Minimum Qualifications:
  • Current RN licensure in the practicing state.
  • Preferred: Minimum of one year of experience in Case Management.
  • Complies with all relevant professional standards.
  • Completion of Amergis' Competency program when applicable.
  • Current CPR certification if applicable.
  • TB questionnaire, PPD, or chest x-ray if applicable.
  • Current Health certificate as per contract or state regulations.
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training as applicable to the job site.
  • Understanding of patient confidentiality and HIPAA regulations.
  • Strong communication skills with the ability to effectively exchange information with various stakeholders.
  • Proficiency in computer skills is required.
  • Must be at least 18 years of age.
