Summary

Directs administration of Clinic(s) by performing the following duties personally or through the subordinate lead roles. Provides comprehensive care to the assigned patients and serves as direct support staff to the clinicians at the school-based clinic.

Essential Duties and Patient Centered Responsibilities include the following. Other duties may be assigned.

Oversees administration of ongoing clinical programs and promotions with the best interest of the patient population.

Administers fiscal operations such as budget planning, maintaining, and submitting supply orders.

Directs hiring needs and training of personnel.

Negotiates for improvement of and additions to buildings and equipment.

Directs and coordinates activities of medical, nursing, licensed social workers, certified professional counselors, laboratory, clinician staff and services. Also directs and coordinates activities of the call center if assigned.

Aids the Administrative staff and Director of Operations and Quality in administering and implementing Family Circle of Care policies and procedures.

Represents clinic(s) at staff and community meetings, when necessary; as well as promotes ISD programs within each department as directed by the Chief Executive Officer and Chief Operating Officer.

Oversees and acts as liaison between clinicians and medical support staff in the clinic(s).

Leads team huddles to promote clear communication amongst care team and treating clinicians.

Registers intake and exit of patients.

Provides information through the resource center for patients and family members as deemed appropriate.

Administer immunizations and other medications as ordered by the clinician; note clinical information in EMR system.

Obtain and record patient vitals and weight.

Review clinician orders, lab requests, and follow up with patient as needed.

Collect samples, such as urine, blood and sputum from patient as assigned.

Participates in the CPI program for continuous quality improvement and risk management purposes.

Assists with nurse telephone triage, responding to patient telephone inquiries by evaluating and responding to patient’s problems and questions as needed.

Communicate with school nurses, counselors, and administrative staff for collaboration of clinic and school needs.

Attend and provide education for school outreach programs as needed for clinic support.

Maintains compliance, is knowledgeable, and utilizes the guidelines of all state and federal laws and regulations as they pertain to the position including, but not limited to; Health Insurance Portability Accountability Act (HIPAA), Scope of Practice, Accreditation Standards, Occupational Safety and Health Administration (OSHA), Bureau of Primary Health Care/ Health Resources and Services Administration (BPHC/HRSA), National Committee for Quality Assurance/Patient Centered Medical Home (NCQA-PCMH), Centers for Medicare and Medicaid Services (CMS), Clinical Laboratory Improvement Amendment (CLIA), Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and other HRSA related regulatory functions and/or agencies that Family Circle of Care participates in.

Supervisory Responsibilities

Is responsible for the overall direction, coordination, and evaluation of the Call Center, OB/GYN, Pediatrics, and Family Medicine Clinic(s) assigned. Also directly supervises medical support staff, laboratory staff, licensed social workers, and certified professional counselors. Manages Lead LVN’s or RN’s who assist supervising and coordinating the day-to-day Clinical Medical Support Staff, Laboratory, Licensed Social Worker, and Certified Professional Counselor responsibilities. Carries out supervisory responsibilities in accordance with Family Circle of Care policies and applicable laws. Responsibilities include interviewing, maintenance of staff hiring needs and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing employee and patient complaints and resolving problems.

Qualifications

Education and/or Experience

Associate’s degree from two-year college or technical school in Vocational Nursing; prefer one-year clinical nursing experience.

Healthcare leadership or related field preferred; one year’s experience in front line supervisory role.

Computer Skills

To perform this job successfully, an individual should have knowledge of Microsoft Word Processing software, Microsoft Excel Spreadsheet software. ADP Payroll systems and Electronic Medical Record System Database software experience preferred.

Certificates, Licenses, Registrations

LVN License in the state of Texas required. Responsible to maintain license and obtain all professional CEU’s in compliance with Licensure Boards and the City, State, and Federal Regulations.

Current BCLS Healthcare Card with renewal every two years required.

Equipment

Point of Care Testing with glucose meters, blood pressure monitors and other PCOT equipment.
