We are a Long Term Care facility in Westminster that specializes in taking care of the chronically ill, stable mentally ill, brain inquired and resident with behavioral issues. We will train you and give you the experience you need to care for residents in a long term setting with a specialization in Behavioral Health.

Activities Assistant/Caregiver

A Park Forest Activities Assistant/Caregiver assists individuals and the nursing team, who have difficulty performing basic day-to-day activities. Caregivers within our community are expected to document behaviors, assist with cleaning, general maintenance and up-keep of facility and residents, perform one-on-one duties, assist with activities as needed, escort residents to appointments, and support the nursing team to care for our residents.

The person is expected to know and adhere to all basic facility policies and procedures, and respond appropriately during emergency situations. The person is expected to maintain the resident’s environment, perform general unit upkeep, order and maintain unit equipment, and inform appropriate supervisors of supply needs without direct supervision. The person is expected to use accurate verbal and written communication, and to document resident care using the appropriate platforms.

The AA/Caregiver works under the general supervision of the Nurse and Activities Director when caring for residents in stable condition. The person will also work under the direct supervision of the Registered Nurse in the care of the acutely ill and/or complex resident. The AA/Caregiver receives specific work assignments which are both written and oral. The caregiver is expected to demonstrate evidence of efficient time management by completing the work within the scheduled time.

Major Responsibilities

  • Provide daily support to nursing staff via monitoring and documenting the placement of residents.

  • Document activity participation from residents both on paper and electronically.

  • Administer and direct resident smoke breaks

  • Attend appointments with residents as needed

  • Document resident behaviors

  • Assist and direct activities with residents

  • Assist nursing team with one-on-one resident supervision, as needed

  • Provide residents with ice, water, and snacks scheduled and as needed

  • Assist CNA’s with bed making and tidying up of resident care areas, dispose of trash and provide clean linen and supplies to residents daily and as needed.

  • Assist residents with smoking breaks

  • Notify CNAs and Nurses of resident specific cares needing completion.

  • Answer call-lights and address resident needs within scope of practice.

  • Pass resident room trays for meals as needed.

  • Ensure the safety and well being of residents by following mandated reporting policies and procedures.

  • Before exiting the resident room, ensure that safe room set-up is implemented.

  • Answer facility phone calls and direct phone calls to appropriate areas.

  • Assist nursing team with additional tasks needed

Assist in Transportation for Residents:

  • Assist in coordinating transportation services for residents, including medical appointments, outings, and other scheduled events.

  • Collaborate with the transportation director or coordinator to create transportation schedules and routes that meet the needs of residents while optimizing efficiency.

  • Communicate transportation details to residents, families, and staff members in a clear and timely manner.

Safe and Comfortable Travel:

  • Ensure the safe and comfortable transport of residents to and from their destinations, following all applicable regulations and safety guidelines.

  • Assist residents with boarding and disembarking vehicles, taking into account their mobility and assistive device needs.

  • Monitor and address any concerns related to resident comfort and well-being during transportation.

Record Keeping and Documentation:

  • Maintain accurate records of transportation schedules, trip logs, mileage, and resident attendance.

  • Document any incidents, accidents, or issues that occur during transportation, and promptly report them to the transportation director or coordinator.

  • Assist with maintaining up-to-date driver certifications and licenses, as required by regulatory standards.

Resident Communication and Customer Service:

  • Interact with residents in a courteous and professional manner, ensuring their comfort and addressing any questions or concerns they may have.

  • Provide clear instructions to residents regarding transportation arrangements, including pick-up and drop-off times, locations, and any special considerations.

  • Foster a positive and supportive environment during transportation, promoting resident engagement and social interaction.

Collaboration and Teamwork:

  • Work collaboratively with other staff members, including the transportation director or coordinator, nursing staff, and activities staff, to ensure coordinated care and seamless transportation services.

  • Participate in staff meetings, training, and quality improvement initiatives as required.

Qualifications and Skills:

  • High school diploma or equivalent is required.

  • Previous experience in a similar role, preferably in a healthcare or long-term care setting, is desirable.

  • Knowledge of therapeutic and person-centered activity programming principles and techniques.

  • Familiarity with transportation regulations and safety guidelines, particularly related to transporting older adults.

  • Excellent communication and interpersonal skills to interact effectively with residents, families, and staff members.

  • Compassion, patience, and a genuine passion for working with older adults.

  • Basic knowledge of vehicle safety and maintenance is an asset.

  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.

  • Ability to work both independently and as part of a team, adapting to changing situations.

  • Valid driver's license and a clean driving record.

  • Ability to lift and assist residents with mobility needs, as required.
