Case Manager

JOB DESCRIPTION
QUALIFICATIONS
- High School Diploma or equivalent and 2 or more years’ experience working with human service agencies/programs BA or BS degree in social work/human service degree or related field encouraged.
- Working knowledge of modern office communication systems: e-mail, voice mail, computer systems, and ability to operate various office equipment such as fax, copy machine and calculator.
- Possess strong verbal and written communication skills as well as leadership and motivational skills.
- Bilingual (English/Spanish)
RESPONSIBILITIES
- Respond to emails, calls, and messages within 2 business days
- Complete assessments of clients’ dimensions of wellness
- Contact clients upon assignment to a therapist to provide an overview of agency services and follow up with clients every 60 days thereafter
- Assist clients with Medicaid renewal, as needed
- Identify and connect clients to resources
- Maintain complete and accurate case records completing all documentation in a timely manner
- Assist in agency outreach
- Other duties as assigned
