JOB DESCRIPTION


QUALIFICATIONS


  • High School Diploma or equivalent and 2 or more years’ experience working with human service agencies/programs BA or BS degree in social work/human service degree or related field encouraged.
  • Working knowledge of modern office communication systems: e-mail, voice mail, computer systems, and ability to operate various office equipment such as fax, copy machine and calculator.
  • Possess strong verbal and written communication skills as well as leadership and motivational skills.
  • Bilingual (English/Spanish)


RESPONSIBILITIES


  • Respond to emails, calls, and messages within 2 business days
  • Complete assessments of clients’ dimensions of wellness
  • Contact clients upon assignment to a therapist to provide an overview of agency services and follow up with clients every 60 days thereafter
  • Assist clients with Medicaid renewal, as needed
  • Identify and connect clients to resources
  • Maintain complete and accurate case records completing all documentation in a timely manner
  • Assist in agency outreach
  • Other duties as assigned
