Job Summary

Community Crisis Services is seeking a dedicated and compassionate Case Manager to provide direct services to shelter residents, including crisis intervention, advocacy, and case management. The successful candidate will play a crucial role in empowering survivors through compassionate care, advocacy, and education, ensuring their journey to recovery and independence.

Key Responsibilities:

  • Conduct comprehensive assessments to identify the needs and strengths of shelter residents
  • Develop and implement individualized service plans in collaboration with shelter residents
  • Provide crisis intervention, safety planning, and emotional support
  • Assist shelter residents in accessing community resources, including housing, legal services, healthcare, and employment
  • Advocate on behalf of residents with various agencies and service providers
  • Facilitate support groups and educational workshops
  • Maintain accurate and confidential case records and documentation
  • Participate in regular staff meetings, trainings, and professional development opportunities
  • Collaborate with shelter staff and community partners to ensure comprehensive and coordinated care

Requirements:

  • Bachelor's degree in a social science discipline (Psychology, Sociology or Social Work) or related field
  • A minimum two years of case management experience
  • A minimum of two years' experience working with at-risk clients in a direct service setting
  • A minimum of two years' experience working with clients who have a history of domestic violence, addiction, mental health issues and/or the displaced population (homeless)
