SUMMARY OF RESIDENT CARE COORDINATOR POSITION

RETENTION BONUS $3,000

Waneka Park is looking for a caring, compassionate and kind Resident Care Coordinator. We strive to provide care to our residents that exceeds industry standards and we are looking for a Resident Care Coordinator that wants to join that mission

The Memory Care Resident Care Coordinator is responsible for the daily coordination of the health care needs of the residents in the memory care community as well as oversight of the care partners.

The ideal candidate for this role will have a passion for serving and caring for the elderly population and the desire to build a genuine rapport with those that they’re charged to care for.

RESPONSIBILITIES

  • Performs resident care coordination and communication with outside agencies and medical providers.
  • Assures that resident care plans are read and followed by care partners.
  • Reviews care partners schedules and assures compliance with staffing plan daily.
  • Reviews, supervises and assists with resident care, including resident meals, as needed.
  • Coordinates resident appointments. Occasionally attends appointments with the residents or assist on the floor as needed when other staff members are out of the facility for an appointment.
  • On Call and assist with covering open shifts as needed.
  • Assist with scheduling staff

GENERAL REQUIREMENTS

  • High school diploma or GED required.
  • QMAP Certification
  • CPR certified from accredited organization
  • Previous work with the elderly population is desirable, but not required.
  • Previous work as a Medication Technician is desirable, but not required.
  • Knowledge and understanding of residential care principles and rules in the state where employed.
  • Ability to create strong working relationships with diverse populations and leadership styles.
  • Excellent written/verbal communication and customer service skills.
