RN MDS Coordinator

The State of Indiana is seeking a skilled RN MDS Coordinator to join our team. As a key member of our healthcare team, you will be responsible for coordinating the care of our residents, ensuring that their needs are met and their quality of life is maintained.
Key Responsibilities:
Conduct resident assessments and develop individualized care plans
Coordinate with healthcare teams to ensure that residents receive high-quality care
Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Collaborate with other departments to ensure seamless care delivery
Requirements:
Associate degree in Nursing from an accredited college
License to practice nursing in the State of Indiana as a Registered Nurse
CPR certification
Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records
