East Bay Brain & Spine is seeking a compassionate and organized Care Coordinator to join our dedicated team.


About the Role:


In this role, you will be the bridge between patients, physicians, and other healthcare providers. You will work diligently to ensure patients receive coordinated care throughout their healthcare journey. The position is at an outpatient neurosurgical office with patients with history of strokes, aneurysms, carotid stenosis, arteriovenous malformations, and brain tumors. There is growth potential in the position to expand into surgery or procedural scheduling and authorizations. Your responsibilities will include:

  • Patient Communication: Effectively communicate with patients by phone, email, and in person to schedule appointments, answer questions, and provide updates on their care plan.
  • Care Coordination: Collaborate with physicians, nurses, specialists, and other healthcare providers to place orders and obtaining authorizations.
  • Appointment Management: Schedule appointments, ensure timely referrals, and manage patient flow to minimize wait times.
  • Documentation: Maintain accurate and up-to-date patient records in the electronic health record (EHR).
  • Quality Assurance: Contribute to quality improvement initiatives by monitoring patient outcomes and identifying areas for improvement.
Requirements:

· Excellent customer service/interpersonal skills

· At least two years experience as a medical assistant

· MA Certificate

· Medical terminology

· Excellent verbal and written communication skills

· Strong computer skills


Required Qualifications:

· Bachelor's Degree


Strongly Preferred Qualifications:

· Electronic Health Record (EPIC)

Pay Scale/Ranges: $30.00 - $35.00 per hour .*
