Job Summary:

Under the direct supervision of the Center Manager and Home Care Manager, responsible for the implementation of home care services for program Participants, including the coordination of home schedules and verification of payroll and timesheets.

Schedule:

Standard operating hours are 8:00 am – 4:30 pm; Monday-Friday. Please note work hours may fluctuate depending on the Participant and business needs, flexibility is required.

Hourly Wage:

Duties/Responsibilities:

  • Home Care Team Leader will complete the Home Care, Safety and Environmental Assessment form at least every 6 months or as trends are identified, which indicates the need for surveillance to be done more often. Completion of all documentation in medical charts

  • Completion of Home Care log.

  • After home visit and assessment are completed, recommendations for home care are made to the Interdisciplinary Team.

  • Ensure home care schedules are staffed appropriately per the Home Care assessment.

  • Notify the Home Care Team Manager of any need of contracted home care.

  • Verify Home Care PCA timesheets, mileage logs and submit to payroll timely.

  • Communicates any issues to PCA Supervisor regarding the need of home care.

  • Fills in for Home Care when unable to obtain coverage.

  • Participates in QA Initiatives when directed.

  • Ordering/Tracking of all Durable Medical Equipment in EMR per Policy and Procedures

  • Completion of all scheduled Care Plans

  • Attends the IDT meeting to represent the need of care in the homes

  • Any other duties directed by the IDT team.

  • Every effort has been made to identify the essential functions of this position.

  • Other duties as assigned.

Required Skills/Abilities:

  • Relates well with people of all ages but has a deep sensitivity and commitment to the elderly.

  • Ability to organize work and follow through on assigned tasks.

  • Excellent verbal and written communication skills.

  • Must possess the ability to make independent decisions when circumstances warrant such action.

  • Must be able to deal tactfully and patiently with personnel, Participants, family members, visitors, government agencies/personnel, and the public.

  • Must have the cognitive ability to process and perform tasks for Participant safety.

  • CPR and First Aid Certification.

  • Must be a minimum of 18 years of age.

Education and Experience:

  • High School Diploma or GED.

  • One year of experience working with the frail and elderly population and at least one year of prior supervisory experience.

  • Must possess a valid PA driver’s license, vehicle, and clean driving record.

  • Good interpersonal skills and the ability to work with geriatric patients and handle unusual interpersonal situations.

Requirements:

  • Requires valid State driver’s license with a clean driving record.

  • Successful completion of background check, physical, drug screening, TB Screening

Physical Requirements:

  • Must be able to move intermittently throughout the workday.

  • Must be able to lift, push, pull, and move a minimum of 25 pounds.

  • Must be able to assist with the evacuation of Participants.
