Summary:

We are seeking a highly motivated and experienced individual to take on the role of Value-Based Care Post-Acute Network Coordinator. This position is instrumental in ensuring the delivery of high-quality patient care within the post-acute network, with a strong focus on driving appropriate length of stay and optimizing patient outcomes. The successful candidate will be responsible for fostering collaborative relationships with Skilled Nursing Facilities (SNFs) and other post-acute care sites, actively monitoring facility performance, and implementing strategies to enhance the value-based care model.


Responsibilities:


  1. Post-Acute Network Management:

- Oversee and manage the value-based care post-acute network, including Skilled Nursing Facilities, rehabilitation centers, long-term care facilities, and other relevant post-acute care sites.

- Identify and onboard new facilities and providers that align with the organization's value-based care objectives and standards.

- Regularly evaluate the performance of existing network facilities, ensuring they meet quality care metrics and compliance requirements.


  1. Quality Assurance and Performance Monitoring:

- Establish and maintain a system for monitoring and evaluating the quality of care provided within the post-acute network.

- Conduct regular site visits to Skilled Nursing Facilities and other care sites to assess compliance with established guidelines and protocols.

- Collaborate with the Quality Improvement team to develop and implement action plans for continuous improvement of care quality.


  1. Value-Based Care Implementation:

- Support the development and implementation of value-based care initiatives and models within the post-acute network.

- Assist in the design and execution of programs that encourage providers to adopt evidence-based practices and improve patient outcomes.


  1. Data Analysis and Reporting:

- Utilize data analytics and performance metrics to track key performance indicators and outcomes within the post-acute network.

- Generate regular reports to showcase the effectiveness of value-based care programs and identify areas for improvement.

- Lead regular meetings with post- acute partners to share outcomes and drive collaboration to ensure quality outcomes


Requirements:


- Bachelor's degree in healthcare administration, nursing, public health, or a related field. Master's degree preferred.

- Minimum of 5 years of experience in healthcare management, with a focus on post-acute care and value-based initiatives preferred.

- Strong knowledge of value-based care models, accountable care organizations, and post-acute care delivery systems.

- Excellent communication, negotiation, and relationship-building skills to collaborate effectively with diverse stakeholders.

- Proven ability to analyze complex data sets, draw insights, and implement data-driven solutions to improve care quality.

- Understanding of regulatory and compliance requirements relevant to post-acute care settings.

- Ability to travel to various post-acute care sites for site visits and meetings.
