Employment Type: Open to Permanent, Contract, Per diem, and Locum Tenens roles


Job Title: Family Nurse Practitioner (FNP)

Location: Phoenix, Arizona (Hybrid)

Pay Rate: $75 - $85/hr on W2, Based on experience, with an option for 1099 available (Higher Rates)

Duties and Responsibilities:

  • Deliver primary care services with a client-centered, harm-reduction focus.

  • Administer medications such as psychiatric, Vivitrol, Suboxone, or Subutex.

  • Refer patients to specialists, hospitals, community agencies, or in-house staff as needed.

  • Maintain accurate and timely records, including medical evaluations and physical examinations.

  • Provide emergency care and treatment for conditions such as anaphylaxis and acute asthma.

  • Work flexible schedules in a hybrid model at multiple locations.

  • Advocate for clients' needs to ensure access to effective care and resources.

  • Participate in on-call coverage to support clients.

Essential Requirements:

  • MSN from an accredited college or university.

  • Completion of a Nurse Practitioner program specific to family medicine.

  • Current DEA License

  • At least 2-3 years experience as an NP in medication-assisted treatment and family medicine.

  • CPR certified within 3 months of hire.

  • Ability to work in a hybrid environment with a stable internet connection.

  • Reliable transportation for mobile care.

  • Licensed with the state of Arizona.

  • Ability to pass a credit check, background check, and drug screening.
