In this role you will:
  • Work with the interdisciplinary team to execute a customized plan of care for each unique patient.
  • Guide the patient through the treatment process, reviewing instructions, and scheduling appointments.
  • Facilitate interaction between patients and their physician and referring physician(s) as appropriate. 
  • Navigate the patient and family across care settings according to the established patient care plan and clinical pathways
  • Utilize effective communication to ensure patient and caregiver(s) have appropriate information, understand and have the capacity and tools to adhere to the individualized treatment plan.
  • Contribute to quality-driven performance improvement initiatives.
  • Assist providers with necessary procedures including breast biopsy

We require:
  • Bachelor’s Degree Nursing
  • Current New York State Registered Professional Nursing License
  • Excellent communication (both oral and written), interpersonal, organizational skills.
  • Consistent professional conduct and meticulous attention to detail.
  • Proficiency in Electronic Health Record documentation 
  • Minimum of two years’ experience in an outpatient medical office setting.
  • Healthcare or Customer Service Experience
  • Patient Navigation Certification (NYCBS will fully support our team member(s) in achieving and maintaining specialized certification.)

Hours: Monday–Friday

Salary up to $55/hr. based on experience. Benefits include Tuition Reimbursement, Health Insurance on day 1 of employment, Dental, Vision, Life Insurance, Short and Long-term disability, 401k Plan, generous PTO time, 8 paid holidays (2 floating), and more.
