The Care Coordinator establishes a positive, upbeat environment building on client strengths. These individuals create an emotionally safe, non-punitive setting that is engaging and nurturing. All progress and significant events are documented and discussed with the Treatment Team, to ensure everyone is on the same page about doing what is in the best interests of our clients.
Anderson Health Services has launched mental and behavioral health programs to provide 24/7 emotional support to clients with a history of complex trauma. The hours for this position are 7am-3pm. Everyone is dedicated to offering high quality, culturally sensitive care to adolescents in need of emotional help and support.
The Care Coordinator at Anderson Health Services is expected to:
- Coordinate and facilitate Child and Family Team meetings.
- Lead the development, implementation, and evaluation of individualized services plans.
- Develop trust and mutual respect with youth/families and their team.
- Assist team to identify needs and create a shared vision.
- Assist teams in identifying, recruiting, and engaging natural supports.
- Enable youth/family and team to take ownership of the Wraparound process.
- Support youth/family in identifying their strengths and building upon them.
- Identify barriers to full participation in the program service process and develop strategies to overcome them.
- Prepare summary documents and final reports.
- Ensure that all required confidentiality and other compliance documents are current for each youth.
- Educate families, providers, and community about the elements of the program and its processes.
- Contribute to and support a positive, team-oriented work environment.
- Complete documentation of services and treatment plans consistent with clinical and administrative policies and procedures.
- Work in collaboration with direct care staff supervisors, clinicians, and the family to build rapport and overcome challenges.
Educational Requirements
Bachelor’s Degree from an accredited university, in a Human Services related field; master’s degree preferred. At least two years of experience providing services for children and families.
Physical Requirements
This position will require a wide variety of physical tasks such as standing, walking, running, stooping, bending climbing stairs, and lifting individuals, which requires strength, coordination, endurance, and agility. In addition, there may be occasional contact with hostile clients, and incumbents may be subjected to physical and verbal abuse while executing physical or protective interventions. The employee must maintain sufficient physical ability to ensure safety, in the event a client becomes violent, and attempts to hurt themselves or others. The employee must be agile enough to control and rapidly extricate themselves and clients from buildings in case of emergency.
Additional Requirements
Must be 21 years and older and have a valid NC driver's license with a good driving record. Must be able to demonstrate effective self-regulation strategies. Must be coachable and willing to learn. All potential job candidates must pass a drug screening and background check.
