JOB OBJECTIVE


The Community Coordinator will work closely with the administrative team and will be expected to allocate approximately 80% of

their time to direct care and 20% to administrative tasks.


Additionally, the Team Lead must possess a valid driver's license, maintain auto insurance, and be willing to engage in

transportation-related responsibilities.


Supervision and Support:

  • Provide direct supervision to a team of DSPs, ensuring adherence to CARF standards and company policies.
  • Conduct regular performance evaluations and provide constructive feedback for professional growth.
  • Foster a positive and inclusive team environment, promoting open communication and collaboration.

Direct Care:

  • Deliver high-quality care to individuals with IDD, ensuring their safety, well-being, and personal growth.
  • Assist in the development and implementation of Individualized Service Plans (ISPs), promoting individualized support and skill development.

Administrative Tasks:

  • Assist in the coordination and scheduling of shifts, ensuring adequate coverage and compliance with regulatory requirements.
  • Maintain accurate and up-to-date records, including client progress notes, incident reports, and other documentation as required.
  • Participate in team meetings, case conferences, and training sessions as needed.

Transportation Responsibilities:

  • Safely transport individuals to and from various appointments, activities, and community outings.
  • Ensure compliance with all traffic laws and company policies related to transportation.

Complete any additional tasks or projects as assigned by the supervisor or

management team.

Requirements:
  • Strong communication and interpersonal skills.
  • Ability to maintain confidentiality and demonstrate empathy and patience.
  • Knowledge of CARF standards and regulations.
  • Valid driver's license and reliable transportation with auto insurance.

EDUCATION & EXPERIENCE:

  • High school diploma or equivalent (bachelor's degree in a related field preferred).
  • Minimum of 1 year of experience working with individuals with IDD.
  • Previous supervisory or leadership experience preferred.

PHYSICAL DEMANDS:

  • Ability to lift up to 35 pounds and perform physical tasks associated with the position.

WORK ENVIRONMENT:


• 80% Field

• 20% Office
