Summary of Position
The Social Worker/Admissions functions as the advocate and care manager for the resident and family. Works with residents, families, and resident support persons in accordance with federal, state, and local regulations to provide emotional and social support and resources to allow the resident to exist and function at their highest potential and independent level. Responsible for ensuring that facilities are at full occupancy. Responsible for upholding the standards, ethics, clinical practice and values of the State of Ohio CSWFMT Board, and McGregor.
Essential Duties
- Performs initial screening and evaluation of residents and families. Develops admitting, quarterly and annual comprehensive psychosocial assessments of the social and emotional needs of each resident in the electronic system.
- Help residents/families adjust to chronic illness, aging and life transitions, possible role changes.
- Educates residents/families on the levels of health care (i.e., acute, sub-acute, home care), entitlements, community resources, and discuss advance directives to obtain wishes regarding those decisions.
- Promotes communication and collaboration among health care team members.
- Daily documentation compliance and submission of client related interactions and treatment in accordance with facility, MDS, state and federal compliance rules and policies.
- Gather sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident.
- Schedule Monday - Friday. Weekend coverage as needed.
Qualifications
- Bachelor’s degree from an accredited school of Social Work.
