Behavioral Health Support Services Coordinator

Full-Time, Monday-Friday 8AM-5PM

Job Title: Behavioral Health Support Services Coordinator

Reports to: Co-Occurring Disorder Program Manager

Areas of Responsibility:

The Behavioral Health Support Services Coordinator is part of a multidisciplinary clinical team who serves as the bridge between clinical services, essential community resources and funding sources. Efforts will focus on evidence-based practice and social determinant support of patients with mental health and substance use disorders.

Major Duties:

1. Play an active role in Patient Services using the provider led, team-based Patient Centered Medical Home Care Model.

2. Functioning as an integral team member, assist providers in the provision of care, helping put patients at the center of the health care system, and assist with providing care that is “accessible, continuous, comprehensive, evidence-based, family-centered, coordinated, compassionate, and culturally effective.”

3. Connects patients of the multidisciplinary Behavioral Health department (substance use, mental health and psychiatric services) to available funding sources and contingency management.

4. Maintains and utilizes knowledge of community resources and how to access them.

5. Assess client needs and work to connect them with additional Health Center or CAPWN resources and/or other community resources.

6. Meets with clients to document services needs and assists to identify and facilitate access to potential funding sources.

7. Accurately document services needed, assigned and progress along with follow up contacts.

8. Verify patient’s current payor source and obtain, if required, pre-certification/authorization for services ie: Intensive Outpatient Program.

9. Provide updates on relevant measures to providers or appropriate staff on clients accessing funds, barriers and continuing availability of funds.

10. Utilize the Electronic Medical Record (EMR) to track patient’s on-going care and services.

11. Maintain confidentiality of records and communications following HIPPA privacy and security policies and procedures.

12. Assist department in administrative support as needed, for example, but not limited to, assist with coding of expenses, assist with paperwork and data entry needed for grant management and compliance.

13. Attend meetings and trainings as assigned.

Working Conditions:

  • Clinic/office environment in addition to training, meetings, and community interaction.
  • Well-lit, temperature-controlled clinical setting, with standard medical and office equipment available.
  • Work is frequently dynamic and requires meeting tight deadlines.
  • Work in a highly volatile program in a state of change with many interruptions and interactions.
  • The noise level in the work environment is above average.

Physical Requirements:

  • Ability to frequently bend, stoop, squat, walk and pull drawers.
  • Manual dexterity sufficient to operate office and medical equipment.
  • Must regularly talk, hear and have close vision ability to record and communicate appropriate documents and records.
  • Occasionally lifting and carrying up to 25 pounds.

Minimum Qualifications:

Grade: 13

1. High school diploma or equivalent.

2. Ability to communicate clearly and effectively via oral or written means

3. Ability to remain calm and maintain self-control in the midst of difficult circumstances and emergencies.

4. Ability to adapt to change and respond in a professional manner in challenging situations.

5. Ability to maintain boundaries.

6. Working knowledge of Outlook, Word, Excel and PowerPoint

7. Bilingual incentive pay available.

8. Age 21+

9. Current CPR certification.

10. Valid Nebraska driver’s license with good driving record and insured vehicle.
