Job DescriptionJob Description

The Community Support Specialist role will work for the Social Care Network responsible for the Southern Tier region of New York and will be responsible for functioning as the primary point of contact for organizations providing services in the Social Care Network. This position reports to the Network Operations Manager.

Essential Functions:

  • Operate as the primary point of contact for all matters related to partner relations including handling communication, requests and concerns with community agencies from across the Southern Tier Region in New York State.
  • Responsible for building and maintaining partner and community based relationships, including outreach, on-boarding organizations working with or providing services as part of the Social Care Network.
  • Accountable for documenting, organizing, and updating partner and prospective partner information in the Customer Relationship Management tool. Works collaboratively with associates to ensure documentation processes are adhered too. Ensures partner inquiries are followed-up in a timely manner.
  • Organizes and oversees partner contracting process and contract management, including tracking of contract deliverables.
  • Manage up-to-date, publicly available SCN directory on its website that includes all contracted community based organizations.
  • Collaborate with internal cross-functional teams to meet partner needs and ensure positive partner experience.
  • Coordinates outreach and communication to organizations in the Social Care Network. Uses Care Compass Social Care Network tools and resources to inform partner communications and meetings.
  • Monitor and report progress and results to ensure that initiatives you are working on meet expectations and deadlines. Manage and communicate ongoing changes in tasks, goals, or performance.

MINIMUM REQUIREMENTS:

  • Education: Bachelor’s degree required or 5 years equivalent experience managing and monitoring organization/partner relationships.
  • Excellent organizational skills and demonstrated expertise in managing relationships.
  • Possesses strong communication skills both written and verbal.
  • Exceptional ability to build and maintain collaborative working relationships with internal and external stakeholders.
  • Exceptional customer service and organizational skills, and an ability to prioritize.
  • Proficient in planning & time management
  • Ability to work independently with minimal supervision
  • Skilled with personal computer programs such as Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint, etc.)
  • Facilitation of meetings either in person or via virtual platforms such as Zoom

WORK ENVIRONMENT:

Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. There may be some travel required for partner and/or network meetings.
