We are thrilled to announce our recruitment for a Community Health Worker to join our highly experienced case management team. We are seeking an experienced individual to provide intense case management services to our clients who receive primary care at our clinic. Core services provided by candidate may include coordination of case with a multidisciplinary team to ensure treatment compliance and evaluating/anticipating ancillary, wrap-around and support services. The chosen candidate must be focused, able to handle multiple activities at once, have excellent written and oral communication skills, exceptional computer skills and be very detail oriented.

• Experienced providing Community Health Worker services to medically and socially complex populations.
• Experienced providing social support services to people living with HIV/AIDS.
• Experienced working with the LGBTQ+ community.

Summary
Under general supervision, as part of a multi-disciplinary team, provides services relating to improving retention in care for specialized subsets of patients. Responsible for building patient relationships that encourage disease management and connection with healthcare providers. Conducts prevention intervention programs and shares responsibility for maintaining, enhancing and presenting health information and resources related to chronic disease and other relevant health issues. May perform a range of administrative functions required to ensure efficient program operation.
Minimum Job Requirements of a Community Health Worker:
High School diploma or GED. Must complete New Mexico State CHW Certification within 1 year of employment. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.
Duties and Responsibilities of a Community Health Worker:
- Assists clients in their homes, community, or clinic setting. Communicates to clients/patients the purposes of the program and the impact it may have on their wellbeing. Helps patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
- Assist in identifying individuals who would benefit from retention in care strategies.
- Coaches patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions. Motivates patients/clients to be active and engaged participants in their health and overall wellbeing.
- Assists clients with overcoming barriers to obtaining needed medical care and /or social services.
- Provides support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed.
- Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
- Participate in community events, health fairs, and school events to engage individuals to participate in retention in care clinics.
- Provide clear, understandable, current, and accurate health related information to clients; provide information and referrals in a way that demonstrates respect, active listening skills, empathy, and patience.
- Maintain professional growth and development through completion of required departmental competencies.
- Perform miscellaneous job-related duties as assigned.
