Cardiology Consultants of Philadelphia is a professional medical organization committed to the healing mission of Medicine. CCP specializes in providing comprehensive cardiology care and currently has over 35 locations throughout the Philadelphia area. We are currently hiring for a Clinical Coordinator -Registered Nurse, to work at our Corporate Innovation Department in Center City, Philadelphia, PA.

Please note this role is entitled to a $500 sign on bonus.

Responsibilities:

  • Oversee clinical programs including but not limited to remote patient monitoring, chronic care management, care coordination, and transition of care.
  • Work with clinical and non-clinical staff, information technology, and management to continue developing and implementing new programs
  • Identify and screen eligible patients for participation, explain the study/project to patients and obtain patient consent to participate
  • Maintain data systems documenting recruitment, enrollment, scheduling, and progress through the project/initiative in the correct format
  • Basic data analysis (preparation of graphs and tables) to communicate project results/interim data on an on-going basis to project team
  • Assist in the creation and implementation of new clinical workflows and best practices
  • Disseminate information throughout all offices
  • Train staff and physicians as needed
  • Become knowledgeable with EMR system

Requirements:

  • Must have a current RN license, ACLS, and CPR, BSN required
  • Must have at least 3-5 years of experience
  • Must have the ability to travel throughout Philadelphia and surrounding counties between CCP's 37 office locations
  • Cardiology experience is preferable
