Givens Estates is hiring a Social Services Associate to join our Health Center to support our long-term residents by meeting their psychosocial needs. In this role you'll work closely with our residents and their family members, ensuring that our residents get comprehensive care. You'll also help organize social and fun physical events for our residents to participate in. We're looking for someone who is comfortable spending time with our residents, to find out how we can best serve them.

This position is a full-time, working 32 hours a week, Wednesday, Thursday, Friday, and one weekend day.

What you'll do:

  • Assess and care plan the Psycho-Social needs of our residents
  • Discuss residents needs and updates to families
  • Assist in the transition from rehabilitation to long term care
  • Perform both mandated and regulatory MDS assessments
  • Provide Psychosocial assessments for a holistic experience
  • Collaborate with Life Enrichment Staff in planning and organizing fun physical events
  • Participate, provide input and support department in planned meetings
  • Perform other duties or assigned tasks

What you'll need:

  • Associate degree (or higher) in related field
  • Experience working in a social care role
  • Ability to self-direct and prioritize time management
  • Comfortability with hard topics (death, rehabilitation, etc.)
  • Intermediate to advanced skills with Microsoft Office 365 (SharePoint, Word, Excel, and Outlook)
  • Experience with technology assistive devices
  • Experience working in long term care is preferred
  • Valid Drivers License

Compensation: $20 an hour, plus our comprehensive benefits package
