POSITION OVERVIEW

The Kentucky Housing Specialist is responsible for the care coordination of those living with HIV along with the Matthew 25 team to ensure that patients that test positive for HIV/AIDS are connected to housing resources, with vulnerable populations being the most urgent. The Kentucky Housing Specialist will accomplish this by utilizing Matthew 25’s housing programs, HOPWA and TBRA. These programs assist clients who are at-risk for homelessness and those who are homeless by providing case management services.


QUALIFICATIONS:

  • Associate's degree plus experience or Bachelor's degree in related field.
  • Case Management or Case Coordination experience preferred. 
  • Must be dependable and able to work independently
  • Willing to travel the Tri-State area.
  • Be flexible, willing work a fluctuating schedule.
  • Have a valid driver’s license and insurance card on file with Human Resources Manager
  • Understands the importance of maintaining confidentiality; able to maintain confidentiality under HIPAA standards.
  • Must have the ability to exercise a high degree of diplomacy and tact; excellent customer services skills.
  • Sensitive and non-judgmental of diverse lifestyles.
