The Personal Care Aide works closely with the Director and reports directly to them. This position also works closely with the doctor and nurse to ensure that the quality of life is met.

Lincoln Health is a 15-bed Critical Access Hospital co-located with 22-bed Long Term Care Unit. The facility employs 150-200 employees. Lincoln Park Living Center is 12 bed assisted living. The home employs 6-8 employees, with a Director and an Assistant Director in house Monday-Friday and on weekend emergencies.

ESSENTIAL FUNCTIONS (Personal Care Aide):

1) Provides care as directed by the attending physician to resident requiring assisted living.

2) Reports observations and other pertinent information relate to the care of the resident.

3) Documents objective information related to resident care in the ADL, as well as assisting in the ADL’s with each resident.

4) Provides services that support the care delivered to the resident.

5) Provides input in the formulation and evaluation of the standards of care.

6) Participates in the development of unit objects.

7) Participates in the identification of staff educational needs.

8) Conducts self in a professional manner in compliance with unit and house policies.

9) Participates in the quality assessment and improvement process and activities.

10) Performs other related duties as assigned or requested.

SUPERVISORY RESPONISIBILITIES:

NONE

QUALIFICATION REQUIREMENTS:

MINMUM PERFORMANCE STANDARDS:

Performance in the following areas is acceptable when:

GENERAL RESIDENT CARE:

1) Residents are identified using identification photos that are posted in each resident white file, located in the office, before administrating any meds or meals.

2) Resident is approached in a kind, gentle, and friendly manner. Respect for the resident’s dignity and privacy is constantly provided. Confidentiality of all resident information is maintained, as well as all other resident rights, posted by the library.

3) All grievance and complaints made by the resident are reported to the Director.

4) Assistance is provided with admissions/transfers/discharges:

a. Room is stripped completely after resident has left

b. Resident is moved to another room

5) Resident rooms are cleaned and kept up daily

a. Vacuuming

b. Dusting

c. Trashes emptied

d. Beds made

e. Fresh Linens

f. Clean and bleach bathrooms

6) Care plans are reviewed on a monthly basis or when conditions of the resident significantly change.

7) Assistance with resident care is provided as directed:

a. Baths

b. Linens changed as needed or on their specific date

c. Daily Cleaning/Deep Cleaning

d. AM and PM care/peri-care is given

e. Vital signs are taken as directed, blood pressure, temps, pulse ox

f. Assisting residents to and from meals

g. Help and observe changes in a resident’s condition or behavior, documentation and proper notifications are given to Director

h. Record and document information in the ADL and MAR

i. Answer phone calls and welcome visitors.

j. Protect the organizations values by obeying HIPPA and privacy issues.

k. Serves and protects Lincoln Health and Lincoln Park Living Center community by adhering to professional standards, assisted living policies and procedures, at all levels; federal, state, and local.

l. Update job knowledge by participating in educational opportunities, in-services, and Relias trainings as scheduled.

m. Provides individualized attention, which encourages each resident’s ability to maintain or attain the highest physical, mental, and psycho-social well-being.

n. Fully understands all aspects of resident rights, including the right to be free from restraints and free of abuse.

o. Protects the personal belongings of each resident.

p. Promotes a home-like environment for residents and guests.

8) Assistance with ACTIVITIES OF DAILY LIVING:

a. Personal Hygiene

b. Dressing

c. Eating

d. Maintaining continence

e. Transferring/Mobility

f. Emotional support

9) Assistance with INSTRUMENTAL ACTIVITIES OF DAILY LIVING:

a. Basic communication skills

b. Transportation

c. Meal preparation

d. Shopping

e. Housework

f. Managing medications

10) Hallways are cleared of equipment and other hazardous items.

11) The house is kept up and in orderly fashion.

12) Dishes and kitchen is always kept up and cleaned.

MAINTENANCE OF SUPPLIES/EQUIPMENT:

1) All equipment is operated in a safe manner.

2) Broken equipment is reported to Director.

3) Vacant rooms are neat in appearance.

4) The home, storage room, and med room are kept clean and tidy.

a. All medication shall remain in the proper cabinets and locked 24/7.

b. Full waste containers are emptied.

5) Medications are double counted per shift by two staff members.

6) Cleaning and maintenance occurs according to schedule.

a. Med room and storage room are checked routinely for supplies to be ordered.

b. Linens and other laundry to be cleaned, dried, and properly put away.

c. Refrigerators are cleaned and organized once a week and freezers once a month.

7) The resident’s environment is always safe and clean .

ADHERENCE TO FACILITY PROCEDURES:

1) Procedures are performed according to method outlines in procedure manual.

2) Body substance precautious and other appropriate infection control practices are utilized with all activities.

3) Safety guidelines established by Lincoln Park Living Center are followed.

4) All incidents observed are reported on the shift that they occur with the proper paperwork filled out.

DOCUMENTATION:

1) All entries on charts, notes, ADL, MAR, progress notes, etc., are recorded in an informative and descriptive manner.

2) Vital signs are recorded, intake of food, activities, and other documentation is recorded on the appropriate forms.

COMMUNICATION:

1) All changes in a resident condition are reported as soon as possible to the Director.

2) Staff meeting are attended.

3) Resident, family, and staff problems and concerns are reported to the Director.

PROFESSIONALISM:

1) Decisions are made that reflect knowledge and good judgment.

2) Awareness of own limitations is evident, and assistance is sought when necessary.

3) Responsibility is taken from own professional growth.

4) Attend and participate in educational classes and programs, trainings, and other activities or classes.

5) Follow work schedules and complete assigned tasks according to the established systems, as well as the policy and procedures of the home.

6) Notify Director/Assistant Director and find your own coverage if you are unable to work due to sickness, appointments, weather, or vacations.

7) Organizational ability and time management is demonstrated.

8) Confidentiality of resident is respected at all times.

9) Cooperate with inter-departmental personnel to ensure that the needs of the resident are being met and maintained.

HUMAN RELATIONS:

1) A positive working relationship with residents, visitors, and facility staff is demonstrated.

2) Authority is acknowledged and response to the direction of supervisors is appropriate.

3) Time is spent with resident rather than other personnel.

4) Co-Workers are readily assisted as needed.

WORKING CONDITIONS:

1) Works in the home, resident rooms, kitchen, etc.

2) Sits, bends, stands, lifts, and moves intermittently throughout the work hours.

3) Subject to exposure to infectious waste, diseases, conditions, blood, and body fluids.

4) Subject to frequent interruptions.

5) Attends and participates in continuing education and in-services.

6) Communications with other staff, Director, and Assistant Director.

7) Is involved with the residents, staff, visitors, etc., throughout the work hours.

8) Must return calls and/or text messages in a timely matter when Director or Assistant Director needs to contact you on issues at Lincoln Park Living Center.

SPECIFIC REQUIREMENTS:

1) Must be able to relate information concerning a resident’s conditions.

2) Must be willing to work effectively with professional and non- professional personnel.

PHYSICAL AND SENSORY REQUIRMENTS:

(with or without the aid of mechanical devices)

1) Must be able to move freely throughout the workday.

2) Must be able to speak and write the English language.

3) Must be able to cope with the mental and emotional stress of the position.

4) Must be able to see and hear, or the use of prosthetics that will enable the senses to function adequately, to ensure that the requirements of this position can be fully met.

5) Must be able to function independently, have personal integrity, flexibility, and the ability to work effectively with residents, personnel, and support agencies.

6) Must be in good health and demonstrate emotional stability.

7) Must be able to relate and work with the disabled, elderly, dementia patients, and at times, hostile residents.

8) Must be able to lift, push, pull, and move a minimum of fifty (50) pounds.

9) Must be able to assist with the evacuation of residents in emergency situations

Requirements:

1) Graduation from high school.

2) Completion of certificate program in QMAP and/or 40-hour ALF certificate.

3) Previous caregiving and/or director experience.

4) Able to speak and read English.
