Licensed Clinical Social Worker (LCSW)
(Exempt)

The job duties the employee will be expected to perform will be the following:

  • Provide holistic psychosocial support and behavioral health counseling/therapy to patients in their homes and in assisted living communities.
  • Monitor and coordinate community resources for patients with social economic needs.
  • Coordinate treatment plans for patients with mental illness or other barriers to improve health outcomes.
  • Communicate with team members, providers, patients, and the community in a timely manner to ensure continuity of care.
  • Provide excellent customer service.
  • Collaborate with patient/family and other team members in coordination of patient care.
  • Comply with all medical record standards and policies.
  • Provide timely and appropriate documentation in accordance with Medicare guidelines.
  • Any other duties as assigned in response to the growth of the company and in recognition of employee strengths

Education:

  • Masters in Social Work
  • Florida License Required (LCSW)

Experience:

  • 2 years of clinical counseling
  • Medical Social Work experience preferred
  • Telehealth/home visit experience preferred
