Youth Intervention Services Counselor

PURPOSE:
To provide community counseling services to include outreach, intake, assessment, interventions and referral services for both individuals and groups.
KEY RESPONSIBILITIES:
- Conduct intake assessment, psycho/social assessment, suicide risk assessment.
- Open an average of six (6) new clients a month.
- Provide crisis counseling and referral linkage to individuals and families.
- Provide school based and/or “in-home" crisis counseling as needed.
- Develop service plans in collaboration with clients and enter into Case worthy.
- Address problems identified on the service plan with clients and families.
- Responsible for client files and accurately recording sessions, meetings, phone calls and other pertinent information related to the client and family through progress notes and other documentation.
- Provide group counseling activities and social skill instructions.
- Participate in community outreach activities and document all outreach on appropriate form and submit back-up documentation for outreach to Operations for upload in Case worthy.
- Participate in professional development activities, association meetings and trainings, as required.
- Receives case and file review supervision from the Program Manager.
- Participate in case staffing process and truancy court when needed.

LIST OTHER MINIMUM REQUIREMENTS:
Possess valid Florida Driver’s License in good standing and be insurable by the agency’s current insurance carrier. Must meet state criminal background check requirements.
EDUCATION AND EXPERIENCE:
Bachelor’s degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field; and two (2) years of professional experience in the field of child welfare or social services.
