SUMMARY

TVC Case Manager for the Homeless Veteran Support and General Assistance Program is responsible for assisting homeless veterans, their dependents and surviving spouses in the State of Texas with rapid re-housing and supportive services to eligible clients. He/She will assess, develop, and coordinate a plan to assist each individual. Each TVC Case Manager will outreach for eligible clients at shelters, job fairs, community resource events, and may utilize social media to alert the communities to services available. Each TVC Case Manager will have individual, specific, and measurable goals to meet monthly and annually. The goals will include the number of enrollments, number of support services provided, meeting budgetary goals, group presentations, providing and receiving satisfaction surveys from clients, and coordination with other community resources. The TVC Case Manager must always coordinate their work efforts with other members and/or other programs of the organization, to assure non-duplication of services, provide maximum opportunity and benefits to the client, and always support the integrity and good name of the organization. TVC Case Managers are expected to be sensitive to the needs of these homeless and unemployed veterans and all individuals and must at all times treat the clients with respect and professional care. TVC Case Managers will be responsible for maintaining detailed descriptive case notes on every client, complete and submit required weekly, monthly, quarterly, and annual reports on a timely basis, and perform other program tasks as assigned by their supervisors.

JOB DUTIES & RESPONSIBILITIES

  • Provide case management support to the veterans, surviving spouses and dependents that are experiencing homelessness and that apply for services.
  • Prepare and submit support service requests as needed for transitional housing, utilities, transportation, assistive technology, household goods, pantry items, food, shelter nights, funds to pay for beneficiary ID documents, and other approved supports.
  • Prepare and submit MIS data information on the client cases as required.
  • Responsible for processing support services requests, quotes, and collecting verification receipts for all expenses.
  • Maintain clients’ files, including documentation, case notes, and follow-ups. Responsible for preparing all applicable correspondence related to the client.
  • Responsible for achieving individual goals assigned.
  • May participate in preparing documents, reports, literature, or presentations that will require computer skills and knowledge of software programs including Word, Excel, and PowerPoint.
  • Must understand the nuances of the homeless and veterans’ community and are expected to always demonstrate sensitivity and patience with the clients.
  • Confidentiality of client information, donors, team members and the AGIF-NVOP. Such information is to be disclosed on a need-to-know basis only.
  • Responsible for being sensitive to the needs of program participants and must always treat clients with respect and professional care.
  • Other duties as assigned by supervisor.

JOB REQUIREMENTS

  • Must have a Bachelor's Degree from an accredited college or university and at least (3) years’ experience in a responsible position interacting with veterans, dependents of veterans, surviving spouses, low-income persons or families, and individuals experiencing homelessness, specifically performing as a case manager in temporary and/or permanent housing, providing support services and coordinating with community resources to assist the individuals and families or related field preferred
  • Or a minimum of an Associate’s Degree from an accredited college with a minimum of (5) years’ significant experience in related work experience.
  • Must have transportation and a valid Texas driver’s license.
  • Must also provide proof of current vehicle liability insurance.
  • Must pass a drug test and background record check.

  • Must be able to effectively communicate verbally and in writing.

  • Professional and productive demeanor, as well as ensuring that actions are supportive of others.
  • Must be able to make independent decisions in coordination with service objectives.

PHYSICAL DEMANDS

WORK ENVIRONMENT

While performing the duties of this position, the employee will conduct office work, meet with clientele, and team members, and at times travel offsite to conduct business. Employee may be exposed to varying weather conditions. Employee will also be required to work in varied office and residential environment based on assigned caseload. Employee will be able to handle and de-escalate situations that may arise while at work. Employee will be required to recruit clients in traditional work environments such as job fairs and while networking at community events, as well as recruiting homeless clients where they may reside such as shelter homes, streets, and encampments.

TRAVEL

Primarily local during business day although some out-of-area travel may be expected via airline or automobile based on the discretion of management.
