Role and Responsibilities
The Case Management Team Lead will organize, monitor, assist and ensure that all services delivered by their case management team meet all contractual requirements and follow minimum standard guidelines. This includes providing a high level of oversight to the implementation and documentation of case related tasks. Using advanced knowledge in the area of child welfare, the Case Management Team Lead will collaborate with the Case Management Director by providing input and recommendations in decisions on the overall management and delivery of services to children and families on their team's caseload. This position will coordinate and facilitate activities that work towards moving the child's case to permanency. The position will require a combination of administrative/office duties, training ability, monitoring file compliance and field/travel related work.
Duties
- Responsible for mentoring and supporting case management services as required by external contracts and licensing regulations. To include conducting and documenting weekly 1:1's with each individual on their team.
- Ensures the coordination of admissions, case planning, and discharges of child/youth in accordance with SSCCs, MS and RCCL.
- Works closely with various professional agencies with which Divinity Family Services contracts in a timely and efficient manner.
- May act as a Divinity liaison during licensing inspections and contract audits in Region 7.
- Responsible for training and support of new employees, assisting in planning and directing work; coaching and developing staff. May also be involved in the hiring process.
- Supports the strengthening and management of the culture and practices that reflects the organization's mission and core values and fosters high engagement all aspects of DFS
- Reviews documentation for accuracy, good case planning and implementation of the child's treatment plan. Including but not limited to PARs, service plans, therapy notes and case notes.
- Monitors and assists case managers in reviewing that all necessary documentation is collected and filed to assure compliance with MS, Contracts, RCCL and SSCCs.
- Meets all compliance standards and deadlines according to MS, Contracts, RCCL and SSCCs.
- Close communication with Case Management Director on all concerns, issues, IRs, SIRs, etc. with families and children.
- Provides statistical data to Case Management Director for agency compliance weekly and monthly.
- Field visits with team members. One visit per CM on their Team every 6 months for quality of care evaluations. Additional field visits as needed or directed.
- Monitors case managers caseloads and may provide recommendations on home assignments.
- Prepares Performance Plus Reviews. May also be involved in the creation and implementation of corrective documentation such as record of conversations, write-ups, and professional improvement plans.
- Will conduct in-person team meetings twice monthly at the direction of the Case Management Director.
- Will provide office coverage to the San Marcos office once a week on Wednesdays.
- Approve PTO requests and helps create coverage plans and provide plans to Case Management Director.
- Provide emergency and on-call services as needed to their team.
- Maintain a caseload, as needed.
- Must sign and agree to abide by the "Divinity Family Service's Leadership Creed."
- Other duties as assigned.
Qualifications and Education Requirements
• Bachelor's degree in social work or related field
• Previous work in foster care system or understanding of the Texas DFPS system is preferred
• Working knowledge of Minimum Standards for Child Placing Agencies is preferred
• Bilingual preferred
• Leadership experience preferred
Conditions of Employment
Employee must have and keep a valid Texas driver's license. DFS will also conduct criminal background checks and reserves the right to conduct drug screenings at random. The employee must meet the minimum age requirements for full-time work in accordance with Texas Child Labor Law and the Federal Fair Labor Standards Act. Work weekends and evening hours when necessary. Employee must always maintain working cellular phone service for on-call and after-hours availability. Adhere to all current federal & state laws, as well as Divinity Family Services policies and procedures, regarding the safeguard of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPPA). The employee may be required to stand, walk, sit, use hands to finger, handle, or feel; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl and talk and hear. On rare occasions, the employee may have to lift up to 30 pounds.
