Our client, specializing in Short-Term Rehabilitation, Skilled Nursing, Long-Term Care, Hospice Services, and Respite Care, in Amesbury, MA has multiple openings for LPN's This is an excellent opportunity for long-term work and OVERTIME Apply for this exciting 15+ week assignment, full-time hours and overtime

Responsibilities:
• Assess, care plan, evaluate and coordinate a resident’s care in cooperation with the interdisciplinary team.
• Document residents’ condition in the medical record concisely and accurately
• Communicate between shifts any changes in resident condition
• Medication and Treatment Administration
• Supervision of CNA, and other specialized support staff
• Ensures patient care assignments are consistent with staff competencies, scope of practice and the needs of the patients
• Participates with the interdisciplinary team in developing, implementing and evaluating ways to achieve patient goals and minimize re-hospitalizations
• Contacts attending physician to obtain orders as indicated (admission, change in condition, etc)
• Ensures that physician orders are followed as prescribed
• Oversees medication management to ensure adequate supplies and that all medications are handled in accordance
• Ensures that patients attending physician and family or responsible party are promptly notified of any significant change in the patients’ health condition
• Creates an environment that is respectful, team oriented and responsive to the concerns of staff, patients and families
• Ensures communication between licensed nursing staff and C.N.A.’s during and between shifts
• Ensures complete and prompt reporting of incidents, accidents and adverse events to Executive Director and Director of Nursing
• Investigates patient/family complaints and takes appropriate actions to bring to resolution

Qualifications:
  • Up-to-date Immunizations
  • BLS Certification
  • Active MA LPN License
Shifts Available:
• 3 PM – 11 PM
• 11 AM – 7 PM

Pay: $41.39 - $ 46.63 / hr
