Status: Full Time Days

Shift/Hours: 8am-4:30pm, No Weekends

What do I need as a Social Services Manager with Messiah Lifeways?

  • Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
  • One year of supervised social work experience in a health care setting working directly with individuals.
  • One year of Long-Term Care experience preferred.
  • One year of Supervisory experience preferred.

What will I do as a Social Services Manager with Messiah Lifeways?

  • Supervise Nursing Care Social Workers, coordinate social work responsibilities, and update Social Workers.
  • Coordinate communication between Social Workers and the Welcome Center throughout the levels of care.
  • Provide nursing casework to residents and their families as social and emotional problems or needs arise.
  • Help residents and families to understand, accept, and aid in following
  • recommendations planned to restore residents to optimum social, emotional, and physical adjustment within their capacity.
  • Assess residents' social needs, participates as a team member in devising resident's plan of care, and lead care planning meetings on behalf of residents with staff from this facility and/or other community agencies.
  • Prepare Social Service Initial Evaluations, PTSD evaluations, keep Social Service Progress Notes, and Discharge Plan on nursing residents, including the nature of social needs and how they are being met.
  • Complete sections C, D, E, Q & V of the MDS and care plan according to resident interviews and care area assessments.
  • Participate on various committees planning facilities programs as well as policies and procedures; observe current policies and inform administration of gaps in programs.
  • Implement and/or oversee admission procedure and orientation of residents and their families.
  • Provide information and referral service to the resident families as a part of discharge casework.
  • Responsible for oversight and proper implementation of all aspects of discharge planning.
  • Coordinate all efforts to assure a safe and proper discharge.
  • Participate in determining appropriate level of care for residents.
  • Advocate for residents' rights.
  • Keep up to date with legislation, trends, ideas, resources, and facilities related to field/
  • Lead a Support Group, as needed, for the community and families of residents.
  • Act as a resource person regarding long-term care, the aging process, and the complications of aging to various community groups and all staff.
  • Follow emergency procedures and respond rapidly and appropriately to emergencies ensuring the safety of the residents and staff.
  • Follow proper procedures and promote safe work practices in order to maintain a safe working environment.
  • Work cooperatively and productively with others, including residents, families, and other employees.
  • Willingness to work under direction and supervision.
  • Confidentiality regarding resident information must be maintained in strict confidence, as well as other information pertaining to the business of Messiah Lifeways.
  • Listen to and assist residents/families in the completion of grievance/concern forms.
  • Social Services Manager specifically facilitates resolutions of grievances/concerns as the Grievance Officer;
    • Review form for completion of information.
    • Notify appropriate departments to ensure family/resident is contacted.
    • Review grievance/concern form with Assistant Nursing Home Administrator.
    • Log grievance/concern form.
    • Sign/date form and log completion within seven days of filing.
  • Additional duties and tasks as assigned by the Nursing Home Administrator.
  • Responsible for recruiting, interviewing, hiring, disciplinary actions, performance reviews and terminations of the Social Work staff.
  • Create a quarterly family newsletter to be sent to all families of nursing residents.
  • Manage lost/missing item log and handle any reimbursements that result from said missing item.
  • Coordinates and leads psychotropic IDT meetings.
  • Is familiar with policies in relation to social work (ie. grievances, room moves, expectation meetings, etc.).
  • Enrolls residents in dental care services, manage list for visits, follow up with consult sheets.
  • Send referrals for various services such as behavioral health needs, hospice services etc.
  • Is familiar with nursing care regulations as they apply to social work
  • Performs audits of social work activities to ensure compliance and reports findings to QAPI.
