POSITION SUMMARY:

Washington State’s Wraparound with Intensive Services (WISe) is designed to provide comprehensive behavioral health services and supports to Medicaid eligible individuals (herein referred to as “youth”) with complex behavioral health needs and their families. Services are family driven and youth guided, community based and culturally and linguistically appropriate. The goal of WISe is for eligible youth to live and thrive in their homes and communities, as well as to avoid disruptive out-of-home placements while receiving behavioral health treatment services.

Team members coordinate and facilitate the WISe process for youth and families to include advanced care coordination, facilitation of meetings and ensuring the phases and activities of WISe are completed for each client served by Akin in the WISe program. If certified, will perform peer support duties for either caregivers’ or youth/child Akin clients. May provide parent education activities, resource referral and/or positive mentorship to youth/family along with case management in a variety of settings. Access to protected healthcare information (PHI) or confidential identifiable information (CII), both paper and electronic, as necessary, to perform related job duties and responsibilities.

ESSENTIAL FUNCTIONS:

  • Provide services and supports as outlined in the Washington State WISe manual
  • Provides services at Akin offices, schools, client’s homes and/or in the community at locations where clients are known to gather, such as shelters, parks, community centers
  • Supports a wide range of scheduled activities to assist clients in exercising control over their own lives and recovery process (e.g., to promote socialization, self-advocacy, development of natural supports, and maintenance of community living skills)
  • May prepare and facilitate Child and Family Team (CFT) meetings to include developing meeting agenda, guide team process, and facilitate communication for team and clients served
  • Contacts CFT members who are unable to attend a meeting, and gather their input prior to the scheduled meeting date
  • Helps the youth and family to identify and engage potential supports
  • Establishes effective team culture and identifies their needs
  • Supports youth/family integration into their community by getting to know the family history, culture, and resources
  • Ensures all CFT members receive an updated copy of the CSCP
  • Assists the youth and family in exercising their voice
  • Completes documentation by specified timelines including CANS Assessment, Crisis Planning and Progress Notes
  • Performs other duties as assigned by the supervisor
  • Maintains service intensity expectations
  • Participates in regular case supervision

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of family support principles and practice, childhood development and family development
  • Proficient in MS Office
  • Requires a high level of flexibility and accessibility by working at times and locations that ensure meaningful participation of family members, youth, and natural supports
  • Ability to submit accurate billing data according to the program requirements
  • Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community
  • Must have an understanding of the recovery process for mental health
  • Ability to identify as a Certified Peer - well established personal recovery as defined in the WISe manual
  • Understanding of the provision of services to a diverse socioeconomic and cultural population
  • Ability to work as part of a team to help clients achieve their goals
  • An understanding of local community services
  • Ability to establish and maintain relationships with community agencies and service providers
  • Excellent written and oral communication skills
  • Ability to embrace and support Akin’s commitment to fostering an inclusive organization

WORKING ENVIRONMENT:

Work is split between clients’ homes, office, and car. The following list is representative of what employees encounter while performing the essential functions of this job:

  • Moderate noise (i.e. business office with computers, printers, phones)
  • Typical office lighting and temperatures
  • Moderate interruptions
  • Ability to work in a confined area
  • Ability to sit at a computer for an extended period
  • Home visits involve environments that may include pets, residual smoke, and other typical household hazards like dirt and other spills (Note: we do not allow smoking during visits)
  • Travel for visits often
  • Travel for professional development

WORKING REMOTELY:

Working remotely is a benefit of Akin’s flexible workplace, but remote work cannot be offered for all positions. Some positions require daily presence in the office. Remote work arrangements may be adjusted or rescinded based upon business needs.

PHYSICAL DEMANDS:

  • While performing the essential duties of this job, the employee is regularly required to stand, sit, talk, hear, and use hands and fingers to operate a computer and telephone keyboard; occasionally to reach, stoop and kneel
  • Specific vision abilities required by this job include close vision requirements due to computer work
  • Ability to sit on the floor, crouch, or kneel regularly during home visits
  • Ability to lift children if necessary
  • Ability to sit in car frequently
  • Regular, predictable attendance is required

MINIMUM QUALIFICATIONS:

  • 2 years of direct service experience with children and families of targeted population; Or a combination of education and experience that demonstrates the required knowledge, skills, and abilities required
  • Well-grounded in own mental health and/or substance use recovery for at least one year and have continued management of recovery
  • Bilingual in English and Spanish
  • Agency Affiliated Counselor Credential required after hire
  • Must be available to be on call 24/7 for one week at a time every 10-12 weeks
  • A valid driver’s license is required
  • A one-step Mantoux TB skin test prior to start of employment. For those who have previously tested positive, documented proof of treatment from a doctor, or a negative chest x-ray must be provided
  • CPR, First Aid Certifications required (within 30 days)
  • Successful completion of a criminal background check and be capable of obtaining jail clearance
  • Successful completion of driver checks for positions requiring driving on a regular basis (at least three times per week) or drives clients. Must not have more than two driving incidents within the last three years

PREFERRED QUALIFICATIONS:

  • BA degree in education, social work, or related field

SALARY RANGE AND BENEFITS:

The salary range for this position is $21.08-$28.69. The starting salary range for this position is $21.08 -$22.98 DOE for external candidates.

Employees (and their families) are covered by medical, dental, vision, and basic life insurance. Employees are able to enroll in our company’s 401k plan. Employees will also receive 10 days of vacation leave each year, three personal days each year, and will accrue one sick day per month (pro-rated for part-time employees).
