The Licensed Vocational Nurse performs care services that support the Primary Care Clinic, Adult Day Health Center (ADHC Floor) and Home Care under the clinical supervisor of the Director of Clinical Services, Primary Care Provider, Nurse Practitioner and other licensed staff. The LVN maintains a clean, organized and safe environment. Ability to handle a diverse range of situations and responsibilities is the primary duty of an LVN including but not limited to the planning and monitoring of in-home services and for carrying out tasks in the participants residence as assigned as home safety assessments, data gathering and supervisory visits.

RESPONSIBILITIES

  • Participate in IDT and Care Plan Meetings as scheduled.
  • Must be approachable and able to communicate effectively in and around the clinic area.
  • Assist in prepping patients for assessments or procedures during clinic or home visits.
  • Must be able to complete a comprehensive assessment of the participant during pre-enrollment encounter, semi-annual, unscheduled and annual assessments.
  • Perform and complete home care assessments as scheduled.
  • Follow workflow with home care coordinator licensed vocational nurse lead, personal care assistant and supervisor to ensure that participants are allocated enough time and services based on assessments.
  • Involved in the development and implementation of home care planning activities.
  • Must be able to effectively relay and communicate instructions from the physician to a participants or authorized person with or without use of language line or interpreter as needed.
  • Document participant concerns, change of condition and any changes in the participants baselines status to appropriate staff daily.
  • Review, follow up and document any follow-ups and service delivery requests in a timely manner.
  • Must be able to provide support to participants needs with ADLs and IADLs as needed.
  • Enter and concurrently acknowledge pertinent participant information, results and collected data into electronic health record system.
  • Ensuring participants and their families understand discharge or after visit instructions.
  • Ensure proper body mechanics and participants safety with ambulating/transfer, etc.
  • Assist in processing participants needs received through email, phone call or in person.
  • Administer medications specifically directed by the PCP and be able to recognize and provide education to participant of the medication use and any side effects or adverse reaction.
  • Administer oral, injectable medication(s) including immunizations within an LVN scope of practice and prescribed and directed by the PCP.
  • Starting IVs and IV fluids within the scope of practice and certification.
  • Perform accurate vital sign measurements such as blood pressure, temperature, O2 saturation and weight.
  • Basic wound care includes cleaning and bandaging injured areas.
  • Monitor and reconcile all clinic medications for outdates/expiration, quantity on hand and responsible for notifying Director of Clinical Services of any changes for ordering purposes.
  • Monitor and keep an up-to-date record/inventory of the crash cart and its contents.
EDUCATION AND TRAINING
  • Active LVN license in the state of California
  • IV certification preferred
  • CPR certification
  • Minimum of one (1) year of demonstrated successful experience in the quality healthcare field of work.
  • Minimum of one (1) year of documented experience working with a frail or elderly population.
  • Medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant care.

  • Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
  • Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
  • Ability to communicate verbally with an excellent comprehension of the English language.
  • Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
