The Licensed Clinical Social Worker is a qualified professional, who delivers varied social work services to Homecare patients and their families. The LCSW is an integral part of the Homecare Team, who provides initial and on-going emotional, spiritual, psychosocial, assist with psychosocial issues, community education, outreach, and referral support and services.

Responsibilities:
Conducts ongoing reassessments of patient/family needs and counseling as required.
Provides short term crisis intervention and individual or family counseling when indicated.
Provides services, under the direction of a physician (who approves plan of care). Referral to Community resources as appropriate.
Observes, assesses and information regarding psychosocial, emotional, physical, and financial conditions affecting the patient and family. Assist in understanding the significant social and emotional factors related to clients health problems.
Assumes the active role of advocate for the client/family and caregiving unit.
Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
Help the client and his/her family to understand, accept, and follow the medical recommendations and provides services planned to restore the client to optimum social and health adjustment with his/her capacity.
Quality documentation for all client visits, contacts, referral actions, and assessments, including comprehensive psychosocial assessment clearly and concisely in a timely manner, using the agency electronic medical record. RVNAhealth expects that documentation is completed within the required agency timeframe of 24 hours.
Provides ongoing counseling related to issues of death and dying to the client, family and caregivers as needed.
Attends staff meetings, case conferences, educational programs, and other meetings as assigned and appropriate and assigned. Attend peer consultation meetings as mandated by the State of Connecticut.
Participates in the orientation and other education programs as assigned, including meetings and programs in the Community.
Exhibits Homecare philosophy in all job-related roles.
Follow and utilize appropriate infection control and proper bag technique.
Knowledgeable and compliant with all RVNAhealth, financial, and regulatory procedures including but not limited to Corporate Compliance, HIPAA, Medicare and Medicaid Home Health.
Other duties as assigned by Manager.
Establish respectful, positive working relationships with other employees and volunteers in the delivery of client services.

Qualifications:
Master's degree in social work from a school accredited by the Council on Social Work Education.
At least five years of social work experience, after master's degree has been obtained.
One year of experience in Homecare care setting preferred. One year in health care setting required.
Experience with family assessment and in care planning including goals of care conversations as indicated.
Excellent communication skills and ability to work successfully with others during times of increased stress.
Ability to work independently, make accurate, and at times, quick judgments. Ability to respond appropriately to crisis independently.
Acceptance of and adaptability to different social, racial, cultural and religious modes.
Is computer literate and able to document at point of care.
Is able to drive and travel in all types of weather and environmental conditions.
