Qualifications:

  • Licensed as a Social Worker through the Board of Georgia
  • Possess effective communication skills (including strong verbal and written)
  • Possesses a history of building and influencing patient and family relationships
  • Holds a current drivers license, car insurance, CPR certification, and positive driving record. 
  • Hospital, home health, or hospice experience extremely preferred.

Responsibilities:

  • Assess patient/family social and emotional factors to estimate their potential to cope with terminal
    illness and death.
    • Provide psychosocial counseling and support to the patient/family experiencing emotional, social,
    and economic conflict.
    • Participate in interdisciplinary team (IDT) meeting to establish, review, and revise the plan of care
    for each patient, including exchange of information, review of problems, and sharing of
    professional support.
    • Maintain the dignity of the dying patient.
    • Support the patients and family's unique spiritual and cultural beliefs.
    • Provide care consistent with National Hospice and Palliative Care Organization standards of
    practice for hospice programs.
    • Adhere to all Tapestry Hospice policies and procedures.
    • Maintain confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPPA rules and regulations
