Job Summary:
Tapestry Hospice & Palliative Care is a family-owned and operated organization that has been serving North Georgia for over 16 years. We are seeking exceptional individuals to join our team as Certified Nursing Assistants (CNAs) in a hospice care setting.
Key Responsibilities:
- Provide personalized care to patients in accordance with their care plans, working closely with the RN Case Manager to ensure high-quality care.
- Effectively communicate with patients, families, and the interdisciplinary team to ensure seamless care coordination.
- Participate in the development and implementation of patient care plans, ensuring that patient needs are met and exceeded.
- Collaborate with the healthcare team to provide comprehensive care, including vital signs, medication administration, and wound care.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.

Requirements:
- Current CNA license in the State of Georgia
- Current CPR and First Aid certification
- 1-2 years of hospice experience (candidates with experience in assisted living, home health, or long-term care may also be considered)

- Excellent time management skills
