Job SummaryTo assist in developing and maintaining the physical, mental and emotional wellbeing of each person served requiring enhanced care in ICF-ID Services. To provide care to persons served as well as documenting any pertinent information regarding the same and report to the ICF/ID Manager. Implement individualized skill training to assist the person served to attain levels of independence in their current living arrangement.Essential Responsibilities:
  • Provide intensive skill training, through accepted behavior modification and active teaching techniques, following individualized support plans to persons with severe/potential levels of developmental disabilities. Areas of instruction may include behavioral, domestic, personal care, communication (verbal, written and sign), therapeutic recreation community living skills, occupational and physical therapy.
  • Provide input into assessments to identify priority needs, attend Individual Support Plans (ISP) of assigned persons served and assist with developing teaching programs to be included in the ISP.
  • Teach, train, and assist persons served with ISP goals and record properly.
  • Demonstrates ability to implement teaching plans as written and maintains calm and flexibility when confronted with disruptive behavior.
  • Provides a supportive and therapeutic teaching atmosphere for all persons served, always following the agency's policy and providing the best conditions for the general health and emotional and spiritual well-being of persons served.
  • Assists person served who are unable to provide for their own personal care.
