Summary

Advanced Practice Providers (APPs) are nationally board-certified clinicians that provide primary and/or acute care to individuals across the lifespan through assessment, diagnosis, and treatment. APPs at the Village Health Center (VHC) provide primary care to patients both at the VHC and through other intermittent sites (including but not limited to the Street Health Program). APPs are responsible and accountable for the quality of care within the scope of the position in accordance with standardized procedures for patients. The APP manages patients through medical assessment, diagnosis, treatment, case management, and education with focus on social determinants of health. APPs are mandated reporters.


Essential Functions

  • Provides whole-person, trauma-informed primary health care to people experiencing homelessness (PEH) throughout the lifespan.
  • Performs physical and mental health assessments by conducting health histories and physical examinations, to determine the presence of medical or psychiatric disorders and establish an accurate diagnosis. Utilizes critical thinking & clinical reasoning to synthesize & analyze collected data. Develops, implements, evaluates, and alters treatment plans, as needed.
  • Orders, performs, supervises and interprets diagnostic and laboratory tests.
  • Prescribes medication and non-pharmacologic treatments.
  • Identifies psychosocial interventions that could improve both the physical and mental health status of patients.
  • Provide patients counseling and education that support the patient's overall health and treatment plan.
  • Facilitates effective referrals to the appropriate specialty service to enhance care management.
  • Provides ongoing treatment plan management through routine follow-ups, same day visits, and telemedicine appointments to monitor and record patient’s response.
  • Maintains an adequate load of patients as determined by Village Health Center management. Completes and maintains accurate, concise, and timely documentation in the electronic health record.
  • Participation in the Street Health Program, as assigned. Participation in the Medication Assisted treatment (MAT) project expected, at minimum to provide inductions to new patients and refills for patients presenting for follow-up.
  • Demonstrates insight into own strengths, limitations and knowledge deficits. Identifies learning needs and continuously seeks opportunities for clinical advancement, knowledge, and skill attainment. Considers, utilizes and implements guidance provided in chart reviews.
  • Collaborates and coordinates patient care with a multidisciplinary team of family physicians, advance practice providers (APPs), registered nurses (RNs), mental health professionals, and support staff to provide high quality care and approach challenging complex medical cases.
  • Actively participates in ongoing consultations with collaborating physician as needed.
  • Works effectively with multidisciplinary team members and promotes the CREED mission in team interactions.
  • Demonstrates written and verbal communication skills and ability to work directly with people from diverse identity and socioeconomic backgrounds.
  • Demonstrates knowledge and understanding of the California Nursing Practice Act (NPA) as outlined in the California Business and Professions Code starting with Section 2700 with regulations specified in the California Code of Regulations. Maintains proficiency of current evidenced-based practice guidelines.
  • Participates in quality & performance improvement processes to meet quality of care and service standards, as assigned.
  • On-time completion of assigned training and policies.
  • Performs other duties as assigned.

Requirements

  • Master’s degree from an accredited Physician Assistant or Nurse Practitioner program required
  • Valid CA Physician Assistant or Nurse Practitioner certification required
  • Valid CA DEA license required
  • Active National Provider Identifier (NPI) number
  • Basic Life Support (BLS) Certification
  • 1+ year(s) experience as an APP, preferred
  • Bilingual in Spanish, preferred
