Job DescriptionJob DescriptionHealthcare Staffing Professionals has an immediate need for a Community Health Worker to work in San Diego to support the Aging and Independence Services Health Promotion Unit. The Aging & Independence Services Health Promotion Unit is seeking Community Health Workers (CHWs) to assist with the design, implementation, and coordination of CalFresh Healthy Living (CFHL) and Chronic Disease Self-Management Education (CDSME) programs.
CFHL aims to empower low-income older adults to lead healthy and active lifestyles by providing evidence-based nutrition education programs, followed by “Community Change” sessions. The nutrition education program, Food Smarts, features interactive discussions, hands-on activities, lecturettes, and food demonstrations. The Community Change sessions engage participants in creating policy, systems, and environmental changes that aim to improve access to healthy food and physical activity opportunities. CFHL services also include the production of a quarterly nutrition-focused newsletter, as well as tabling at various community events.
CDMSE programs (known locally as Healthier Living) are evidence-based programs designed to improve people’s management of ongoing health conditions. Topics covered include fall prevention, mental health, physical activity, medication management, nutrition, and more. Participants learn skills and techniques for the daily management of their health, including exercise, healthy eating, symptom management (pain, fatigue, sleep, shortness of breath, stress, and depression), weight loss, and communication skills. Core self-management skills taught include action planning, problem solving and decision making. AIS is licensed to offer three curricula (chronic disease, chronic pain, and diabetes) in English and Spanish.
CFHL and CDSME programs are delivered in a small group format (10-20 participants) at community locations such as senior centers, churches, and libraries; housing sites; and online. AIS staff provide quality assurance, technical assistance, and logistical support for both programs.
The positions will start immediately upon acceptance and employment clearance and are initially funded 90 days with possibility of extension.
Pay Rate-
$21.50/hr
Shift:
A typical schedule is Monday through Friday during the regular business hours of 8:00 am – 5:00 pm; however, occasional work outside of these hours for special events and activities, including weekends and/or evenings, may be required. Leaders will work a hybrid schedule consisting of time working from home, at County offices, and in the community

Duties and Responsibilities:
General
  • Coordinate, advertise, and implement programs in-person and online
  • Maintain fidelity while facilitating evidence-based programs
  • Participate, and in some cases, plan and facilitate, community meetings and trainings
  • Establish connections with internal and external stakeholders to identify new host sites and enhance outreach efforts
  • Track program data and submit timely reports
  • Develop outreach and educational materials for programs and events
  • Prepare equipment, supplies, and materials needed to implement programs
  • Abide by County policies related to secure data collection and storage, incident reporting, etc.
Specific to CFHL
  • Support resident-led policy, system, and environmental changes that improve access to healthy food and physical activity opportunities
Desirable Qualifications:
Required:
  • Facilitator certification for Leah’s Pantry’s Food Smarts program
  • Leader certification for Chronic Disease Self-Management program
  • At least one-year experience teaching or facilitating health-related presentations, classes, or programs
  • Exceptional interpersonal communication, presentation, and facilitation skills
  • Effective writing skills, particularly in communicating health-related information
  • Ability to work independently with minimal supervision.
Preferred:
  • Bilingual (English/Spanish)
  • At least one-year experience working with older adults
  • Proficiency in graphic design tools to create visually appealing flyers and newsletters
  • Knowledge of public health, health promotion, community relations, and implementing evidence-based programs
  • Knowledge of or interest in learning about different cultures, beliefs, practices, and traditions, as well as the unique needs of older adults
Additional Requirements:
  • Leaders must utilize their own reliable vehicle when working in the community and are eligible to receive mileage reimbursement of 67 cents per mile.
