Job DescriptionJob DescriptionAs the Primary Care Nurse (LPN), you will provide patient-centered care in close collaboration with the primary care provider, medical assistant and multidisciplinary departments, including but not limited to triaging acute illnesses, patient education, monitoring of controlled medication refills, transition of care from hospitals, and nurse visits.

Salary Range: $31.00 - $36.00

Job Duties

  • Places patient experience at core of all actions.
  • Effectively triages acute patients' complaints.
  • Provides accurate, detailed and effective medical assessments to provide safe, efficient, and accurate nursing knowledge.
  • Supports the patient in improving access to health resources and social services.
  • Supports the primary care team in meeting performance measures and quality improvement goals.
  • Collaborates with all members of the Care Team to coordinate and manage care for the patient.
  • Provide a Culture of Safety by providing safe, high quality care to patients.
  • Performs health screening procedures under the direction of the Provider including lab procedures (e.g., lead screening, hematocrit, immunizations, etc.).
  • Participates in the preparation of patient’s charts. As directed by the Provider, assists with the patient intake and discharge procedures.
  • In collaboration with Director of QI and Population Health, Healthcare Data Coordinator and Clinical Manager, assists in creating and maintaining care management systems to prospectively identify and track patients who are at highest risk of chronic disease, or who are high utilizers of the health care system to assist in population health management.
  • Proactively engages patients in their care with strategies to improve self-management, including assisting patients in identifying and improving upon self-management goals. Provides patient/family/caregiver education and act as patient advocate in the delivery and coordination of patient care.
  • Collaborates with all clinical staff to ensure care coordination elements of a Patient Centered Medical Home are being met.
  • Facilitates coordinated continuity of care for patients, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan, especially as a result of care transitions.
  • Demonstrates teamwork and collaboration with provider and other members of the care team.

Qualifications

  • Current New Hampshire Nursing License required
  • Experience in primary care and ambulatory health care setting preferred
  • Current BLS certification required

This position is eligible for a one-time sign-on bonus of up to $5,000.00.

GSCH offers earned time accrual (15 days in your first year [pro-rated by FTE]), plus seven (7) paid holidays. Eligibility starts at 24 hours/week, with full benefits offered at 32 hours/week and above.
