Overview

Salary: $30-$33 hourly +sign on bonus

Position Summary
As an LPN with Turning Point, you will provide ongoing care coordination for individuals in residential services and work with various interdisciplinary teams . You will be responsible for managing the health and safety needs for a caseload of individuals, and educating staff and the team at-large . Ideally, as a well-trained professional, provide care coordination with little supervision, be compassionate, cool-tempered, and follow health and safety guidelines. Our goal is to promote the patient’s well-being by providing high-quality health care.

Responsibilities
● Monitor patient’s condition and assess their needs, to advocate for the best possible care

● Observe and interpret symptom presentation and communicate changes to the health care provider team

● Maintain internal health care records and case management documentation

● Collaborate with members of the interdisciplinary team to devise individualized care plans

● Develop person-specific, diagnosis-specific protocols

● Participate in interdisciplinary clinical meetings, nursing team meetings, and staff meetings as needed

● Complete state reporting including annual assessments and incident reporting

● Manage multiple, ongoing deadlines to ensure compliance with regulatory standards and standards of care

● Provide ongoing education to paraprofessional staff with regard to various diagnoses and interventions as determined by acuity and case mix

● Participate in an on-call rotation to cover the triage needs of the individuals across the division

● Foster a supportive and compassionate environment

Requirements
● Valid nursing license in MA required
● 2+ yrs of home care or community health experience
● Excellent working knowledge of nursing standards of care
● Strong communication and interpersonal skills
● Be dependable, reliable, responsible, patient, and compassionate
● Outstanding organizational and multi-tasking skills
● Excellent problem-solving skills
● DDS/MAP experience, preferred
● Google suite experience, preferred
