Western Illinois Home Health Care has been serving the community for over 40 years. Our core hours are Monday through Friday, 8A-5P.
Primary duties of this position include:
- Explaining agency services and programs to prospective clients and their families
- Developing a tailored care plan to meet the clients individual needs
- Helping clients and their families to navigate the healthcare system to ensure their care needs are met
- Matching caregivers to clients
- Working with assigned caregivers to ensure that the clients needs are met
- Providing support and guidance to other staff
- Connecting clients and patients to community resources
- Conducting in-person visits to ensure the clients physical, mental, and social needs are met
- Bachelors degree in social work or related field
- Minimum of 2 years experience in a professional setting
- Passion for working with seniors
