We are actively seeking an experienced full-time Social Worker to join our team. Long term care experience preferred; discharge planning a plus.

Responsibilities
- Conduct comprehensive patient assessments to determine individual needs and develop tailored care plans.
- Provide behavioral therapy and clinical counseling to residents in long-term care facilities.
- Collaborate with interdisciplinary teams to ensure the highest quality of patient care.
- Utilize medical documentation and terminology to accurately record patient progress and interventions.
- Implement discharge planning strategies to facilitate smooth transitions for patients.
- Stay up-to-date on HIPAA regulations and maintain confidentiality in all interactions.
