Cumberland Heights Foundation offers a comprehensive benefits program, which includes:

POSITION SUMMARY
The Recovery Care Advocate supports the overall interdisciplinary plan of care for a patient from admission to discharge and through the first year of recovery post-discharge. The Recovery Care Advocate develops a recovery-oriented relationship with the men patients during the treatment experience, fostering a climate of trust and honest communication. The Recovery Care Advocate builds on the recovery work accomplished during treatment by reaching out to the patient post-discharge, at established time intervals.
PRIMARY DUTIES AND RESPONSIBILITIES include the following:
- Meets face-to-face with the patient while in treatment for an initial orientation about the Recovery Support Services Program features and goals.
- Builds patient trust through personal interactions presented with empathy, understanding, open dialog, supportive recovery principles, and a clinically accepted standard of care.
- Consults with patients on their current living environment with a focus on strengths and the identification of existing recovery capital, or assets that can be developed in support of recovery.
- Assists the program coordinator, primary counselor, case manager and other treatment team members by collaborating with the patient and family in support of specific treatment goals, objectives, and discharge plans.
- Conducts informational presentations about Recovery Support Services to groups of patients as needed and deemed appropriate by the program coordinator.
- Develops a mutual agreement with patient, within program guidelines, for follow-up contacts and support post-discharge.
- Conducts scheduled and as needed post-discharge contact with patients, utilizing telephonic, specialized software, and in-person contacts.
- Communicates with family members, other healthcare providers, and referral sources, as well as relevant internal CH staff, regarding the patient’s progress, as releases of information allow.
- Intervenes with appropriate resources/referrals and/or additional services, including crisis management, as patients’ needs require, documenting the same in the designated manner.
- Appropriately documents all personal interactions in the designated manner.
- Documents information gathered in subsequent meetings that presents as germane to patients’ recovery; (i.e.: family, vocational, financial, medical, spiritual) in the designated manner.
- Promotes a safe and healing environment for each patient and family throughout the continuum of care.
- Recommends ways to improve the quality and delivery of services
- Ensures proper handling and distribution of company funds
- Maintains confidentiality of company and patient information.
- Reacts productively to change.
- Performs other duties as assigned.
SUPERVISORY RESPONSIBILITIES
None
- High school diploma or GED is required
- Minimum of two (2) years of recovery experience including current and active application of 12 step principles
- CPRS certification is preferred but not required.
- Valid Tennessee Driver’s License preferred (may use company van to transport patients)
- Basic computer skills and working knowledge of Microsoft (Outlook, Word and Excel)
- Experience with medical software programs preferred and/or ability to learn clinical software programs
- Ability to problem-solve, analyze, and interpret information.
- Excellent organizational skills and ability to prioritize for managing multiple tasks and assignments.
- Ability to adapt to changing circumstances and patient needs in a fast-paced environment.
- Ability to be open and culturally sensitive to a wide variety of patients’ experiences.
- Knowledge of and ability to apply information regarding cultural/age/population specific characteristics to the patients in the assigned area
- Excellent written and oral communication skills with the ability to effectively speak, read and write in English.
- Ability to interact with patients and co-workers in a collaborative and courteous manner while providing guidance, instruction, and/or training.
- Ability to model/teach the 12 Step programs and philosophy.
- Reacts productively to change.
- Ability to deescalate a person in crisis in a calm and supportive manner; work calmly and professionally in stressful situations and ally with people in crisis.
- Ability to present to hostile or disinterested groups.
- Knowledge of signs/symptoms of intoxication/withdrawal of alcohol and common drugs of abuse; knowledge of general options for treatment
WORK ENVIRONMENT:
- Position is on a campus setting that involves walking outside to various buildings and everyday risks or discomforts requiring normal safety precautions.
- Position requires frequent contact with people in crisis, including those who are emotionally volatile and may require additional safety precautions.
- May perform CPR/First Aid as needed (being certified or eligible is required)
- Position is subject to rotating on-call varying shifts, including weekends as well as staying overnight for inclement weather as part of critical staffing, along with some travel to other off-site Cumberland Heights facilities/campus
- Ability to lift up to 20lbs and/or push pull up to 40lbs; move around campus on uneven and rural terrain.
- Ability to speak, hear, see, sit, walk, stand, reach, and use fine/gross motor skills.
