Case Manager Duties and Responsibilities:

  • Operate as a member of the case management team at Agape and within the case management network.
  • Provide compassionate case management and delivery of services to persons living with HIV/AIDS.
  • Assist clients with managing medical diagnosis(s) and chronic health conditions.
  • Develop and implement individualized service plans through on-going assessments.
  • Establish and maintain community referrals appropriate to clients’ individualized goals and service plans.
  • Advocate on client’s behalf as directly related to client’s service plan and benefits.
  • Attend staff meetings, mandated training, and conferences throughout the year.
  • Provide administrative support as necessary.
  • Daily documentation in the electronic state-database of specific communication & encounters with client.
  • Complete various forms of communication with client such as face-to-face, phone, email, text, etc.
  • On-going treatment adherence counseling with focus on ensuring compliance with medication regiment, M.D. appointments, and barriers which may have adverse impact on overall health.
  • Capable of engaging with different and outside providers to support effective care coordination.
  • Become a Certified Application Counselor (trained yearly) to ensure clients can be enrolled in appropriate Marketplace insurance plans.

Case Manager Requirements and Qualifications:

  • Minimum Bachelor's degree in social work, psychology, sociology, healthcare or medical and related fields.
  • Ability to communicate effectively with clients and co-workers.
  • Comfortable working within social services and working with diverse groups of people including culture, race, ethnicity, sexual orientation, gender identities or expressions.
  • Basic competency with Microsoft software and using technology for reporting and documentation.
  • Enjoys working within a team environment and with a variety of people and perspectives.
  • Valid Illinois driver’s license or access to public transportation (or other types of transportation) for meetings, mandated trainings, and conferences held offsite is preferred.
  • Open to cultural humility practices and on-going training specifically in this topic.
  • Participate in community outreach activities and collaboration with other community agencies.
  • Organizational skills to maintain case load and performance measures required by grant.
  • Written and spoken communication skills for documentation.
  • Ability to manage a case load with multiple amounts of files, schedules, appointments, and information relayed.
  • Self-directed and ability to assist clients in decision-making processes.
  • Empathetic and supportive of clients enrolled in the case management program.
